JF&CS is now seeking a full-time Supportive Services Manager.
PURPOSE:
To manage and oversee services provided for family caregivers and individuals through Case Management grant funded programs, the Geriatric Care Management Services, and other related supportive services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Team Supervision and Leadership:
- Lead and supervise a team of professionals, including case managers, geriatric care managers, and possible other supportive services staff.
- Oversee team’s caseload to ensure proper and effective service delivery.
- Facilitate weekly team meetings to promote collaboration and address team needs.
Client Support Coordination:
- Triage new potential clients and assign them to the appropriate geriatric care manager (GCM) based on capacity and skill set to best meet client needs.
- Collaborate with case managers to manage grant-funded case management programs.
Grant and Financial Oversight:
- Work with Administrative Assistant to track and maintain revenues and grant funding for services, ensuring compliance with monthly, quarterly, and annual reporting requirements.
- Manage the program’s controllable cost budget and monitor expenditures.
Data Management and Reporting:
- Work with Administrative Assistance to oversee demographic and outcome reporting for supportive services, including case management (grant-funded, private pay), support groups, educational programs, and memory support programs.
- Work with Administrative Assistant to administer and analyze program surveys to evaluate outcomes and inform reporting.
Program Development and Standardization:
- Develop and implement processes, as needed, to standardize assessment data collection, progress notes, and service delivery across case management and geriatric care management services.
- Enhance and refine the existing Geriatric Care Management Services model to optimize revenue generation.
- Assist Managing Director in exploring and developing new service offerings to create additional revenue streams.
Outreach and Marketing:
- Work with Marketing staff to inform referral sources about revenue-generating services for older adults and family caregivers.
- Partner with community organizations to leverage resources for individuals and family caregivers in case management support programs.
Accreditation and Compliance:
- Maintain all documentation and client files in compliance with CARF (the agency’s accrediting body) regulations and funding requirements.
- Oversee the CARF certification process for the Supportive Services Team, ensuring all standards are met.
Administrative and Documentation Tasks:
- Enter and submit required documentation into SalesForce.
- Create and manage program paperwork and reporting requirements as needed.
Collaboration and Representation:
- Assist the Aviv Managing Director in developing grant proposals and creating new services and partnerships.
- Represent the Supportive Services team in meetings with grant funders and stakeholders for various case management programs.
Professional Development and Participation:
- Attend all required Aviv and JF&CS staff meetings, as well as professional development training and workshops.

EDUCATION and/or EXPERIENCE:
Master’s degree in an area of human services preferred, with related experience working with elders and family caregivers, as well as strong knowledge of community resources. Candidate must have experience supervising others and managing programs.
