To provide each person referred to our service with the opportunity to achieve his/her optimum level of function and to provide quality patient care.


Position:  Full Time, salaried, non-union, exempt.


 JOB FUNCTIONS 

  1.   Evaluate and assess patients physical, social and emotional needs per physician referral.
  2.   Identify and understand the disease process.
  3.   Evaluations are accurate, complete and age appropriate per departmental policy.
  4.   Correlate diagnostic data with patients assessment.
  5.   Interpret evaluation results and develop appropriate functional goals: short term and long term.
  6.   Communicates goals and plans with patient and family.
  7.   Completes discharge assessment. Completes reassessments per departmental policy.
  8.   Treatment follows a written plan based on completed evaluation results to obtain goals.
  9.   Treatments are performed according to department policies and procedures.
  10.   Assess treatment results and modify as needed per patient performance, new or changing diagnostic data, information from   physician.
  11.   Demonstrated ability to establish treatment priorities and deal with the most important areas first.
  12.   Utilize proper patient safety precautions and universal precautions.
  13.   Assist in planning appropriate discharge plans, home programs and discharge evaluations.
  14.   Document all interaction with patients and family, progress/DC notes to physician as needed and all other documents pertaining to patient case.
  15.   Patient’s performance will be documented using specific and accurate terminology.
  16.   Communicates patient care activities to health care team.
  17.   Participates in interdisciplinary team meetings, utilization review meetings and family meetings as appropriate.
  18.   Completes documentation required for billing and completes patient charges.
  19.   Attends in-services as scheduled and shares relevant information with others.
  20.   Presents in-services inter and intra departmentally as assigned.
  21.   Provides patient and family education.
  22.   Participates in community, industrial educational programs as requested, and prepares adequately for them.
  23.   Participate in the “Job Shadow” program and act as a clinical instructor to P.T. and P.T.A. students as requested.
  24.   Assist the Director of Physical Therapy in developing and implementing new department programs.
  25.   Demonstrates independent judgment to determine alternative solutions to problems.
  26.   Consults with others as needed for patient case and operational procedures.
  27.   Recognize his/her own therapeutic role in the rehabilitation team.
  28.   Aware of personal abilities and limitations and seeks guidance as necessary for performance of duties, asks appropriate questions when in doubt.
  29.   Demonstrates care, consideration, and flexibility with patients, families, coworkers, and others.
  30.   Gives and receives feedback, constructive criticism appropriately.
  31.   Communicates effectively with Director of Physical Therapy and staff.
  32.   Shows support for changes and decisions made.
  33.   Takes initiative, responsibility and ownership for individual contribution.
  34.   Relates well with other hospital staff and departments.

  36.   Demonstrates cost-consciousness while maintaining highest level of quality.
  37.   Establish network and seeks information from many sources.

  40.   Accountable to lead, supervise, develop and implement patient care plans
  41.   Implements appropriate safety procedures for each patient.
  42.   Identifies and rectifies sources of potential accidents through daily and ongoing inspections.
  43.   Utilizes universal precautions at all times.
  44.   Ensures that supplies and equipment are available and in safe working order.
  45.   Notifies P.T. Director or House Supervisor of unusual occurrences.
  46.   Demonstrates knowledge and skills necessary to implement procedures in event of internal or external disaster.
  47.   Demonstrates knowledge and skills necessary to implement procedures in event of fire.
  48.   Demonstrates knowledge and skills to use the M.S.D.S.
  49.   Ensures that work area is clean and orderly.

Quality Improvement:  Actively participates in the Hospital-wide Quality Improvement Program, actively supports and implements Department-specific Quality improvement initiatives and projects, recommends process improvement as appropriate, reports any quality issues in service delivery and consistently commits to a focus on quality improvement and organizational excellence. 


Disaster Management:  As an employee of Penobscot Valley Hospital, the position has an inherent role to care for our community members when in need. To this regard, the incumbent will be expected to participate in emergency/disaster preparedness plan participate in emergency/disaster preparedness planning and drills as requested. as requested. as requested. When called upon during a real life disaster/emergency event, the incumbent will be expected to participate in the Hospital’s response to this event, within the scope of professional and personal ability to do so.

                                                         

Requirements

Graduate of an accredited Physical Therapy Program, diploma, M.S., or B.S. degree required. License to practice or eligible to practice in State of Maine.

Valid Maine driver’s license.

Previous patient care experience preferred. CPR/Basic Life Support Certification required.
