SUMMARY: The Program Director is responsible for the day-to-day operations of the Adult Day Health Care Program to include social work, substance abuse, mental health, clinical services and outreach.
RESPONSIBILITIES:
• Responsible for the administrative and overall operation of ADHC Program.
• Supervises all social workers and substances abuse staff & monitors chart documentation.
• Oversee all aspects of psychosocial programming and services.
• Coordinates the development, implementation and evaluation of case management regimen for clients through assessment, care planning, and monitoring care.
• Ensures admission and discharge procedures are consistent with client need.
• Liaison with outside regulatory and governmental agencies as needed.
• Provides counseling to clients in the Adult Day Health Care Program and Amida Care (SNP).
• Leadership role during the annual NYSDOH-AIDS Institute and IPRO survey process; prepare plan of correction, as needed.
• Provides quarterly audits on all ADHC/Amida Care (SNP) charts.
• Conducts weekly groups for clients and completes the necessary documentation.
• Chair QAPI (Quality Assurance and Quality Improvement Activities for quarterly reports.
• Member of the Comprehensive Care Plan (CCP) team and attends weekly meetings.
• Provides case management for a select caseload.
• Revises & updates departmental manuals as needed
• Oversee outreach and marketing to community organizations to acquire referrals
• Performs any other duties as assigned by Program Director.

Master's Degree in Social Work; New York State License LMSW, LCSW preferred; 2-3 years' experience treating people living with HIV/AIDS in a healthcare setting, substance abuse treatment & counseling; knowledge of NYSDOH survey process; excellent writing and verbal skills required; supervision experience a must; Bilingual preferred.
