Description

AMHC has an immediate opening for a Full-Time Adult Behavioral Health Home Coordinator-Community Integration Specialist (BHH Coordinator) in our Calais area. The Adult BHH Coordinator is responsible for assisting clients with long-term psychiatric disabilities to attain the skills and resources necessary to successfully function in the community. The person will emphasize is placed on holistic care using 8 dimensions of wellness: Emotional, Financial, Social, Spiritual, Occupational, Physical, Intellectual, and Environmental.


Qualifications:

  • A minimum of an associate Degree in a Human Services related field is required
  • MHRT-C certification or MHRT-C Provisional certification is required.
  • Personal vehicle use-required.

Responsibilities & Expectations:

  • Establish Compassionate therapeutic relationships with clients and prospective clients demonstrating the Integrity of AMHC
  • Perform Collaborative case management functions, linking, coordinating, and managing the delivery of services to clients with Excellence
  • Provide Respectful, supportive counseling and assisting clients in selecting and weighing alternatives and making a decision with a focus on Service for the client.
  • Coordinate client's physical and mental health needs

Schedule:

  • This is a 40-Hour per week position
