Behavioral Health Provider


We are seeking a Behavioral Health Provider to join our comprehensive primary care team. As a central member of the team, you will work closely with Primary Care Providers, MAs, PSRs, Care Navigators, Care Managers, and RNs to provide excellent care to our patients.

 

Responsibilities:

  • Conduct Screening, Brief Interventions, and Referral to Treatment (SBIRT) for patients presenting to medical appointments
  • Respond promptly to warm hand-offs for patients with Behavioral Health Concerns
  • Regularly consult with Primary Care Team members regarding health status and progress in treatment
  • Provide updated behavioral health diagnoses (mental health and substance use) to Primary Care Team members
  • Collaborate with Primary Care Team members to develop care plans for patients with behavioral health concerns

Schedule: Monday - Friday 9:00am - 5:00pm with a Saturday Rotation 8:00am - 1:00pm

Requirements

Education and Experience

  • A minimum of a Masters degree in Counseling, Psychology or Social Work
  • Must have a valid Independent License in the state of Illinois (LPC, LSW, LCPC, LCSW or Psychologist) 
  • A minimum of two (2) years experience working in a clinical environment
  • Knowledge of legal and ethical issues related to patient’s rights.
  • Good communication and interpersonal skills.
  • Bilingual ability (Spanish) may or may not be required based on individual department needs.

Knowledge/Skills/Abilities

  • Demonstrated ability to collaborate effectively in a team setting.
  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Comfort working in a medical setting as part of a multi-disciplinary team. 
  • Strong communication and business acumen. 
  • Ability to effectively engage patients in a therapeutic relationship, when appropriate. 
  • Experience with screening, assessing, and carrying out treatment plans for common mental health disorders.
  • Working knowledge of differential diagnosis of common mental health disorders. 
  • Working knowledge of evidence-based psychosocial treatments for common mental health disorders.
  • Experience in working with a diverse population base preferred. 
  • Ability to appropriately handle confrontation and conflict. 
  • Ability to work independently and to seek consultation as necessary.
  • Ability to follow organization and departmental processes and procedures.
  • Ability to work in a high stress/fast paced/high work volume environment.


Salary Description
$60,000 - 75,000 annually
