JOB SUMMARY:
We are seeking a highly motivated and compassionate individual to join our team as a Case Manager for Behavioral Health, Maternal Health, Children, etc. In this role, you will be responsible for providing support and guidance to individuals and families in need of behavioral health, maternal health, and children's services.
MAJOR DUTIES & RESPONSIBILITIES:
- Conduct assessments to determine the needs of clients and develop individualized care plans
- Provide counseling and support to clients and their families;
- Coordinate with healthcare providers and community resources to ensure clients receive appropriate care;
- Monitor client progress and adjust care plans as needed;
- Maintain accurate and up-to-date client records;
- Participate in team meetings and case conferences;
- Participate in regular staff meetings, staff training programs, and supervisory sessions, and accept the responsibility for aiding the development of positive team relationships;
- Interview patients to determine their needs and refers them to appropriate Community agencies and services (i.e. housing, legal aid, emergency shelter, domestic violence, employment, adult education, food/clothing, etc.);
- Document as appropriate vitals and other pertinent information in the patient’s chart, including screenings prior to patient visit;
- Service and respond to customer and patient questions, issues, and/or concerns;
- Provide support and provide patients with behavioral health forms, handouts, and screenings;
- Submit HOPE Clinic forms and documentation in a timely manner (improvement activities, response to memorandums, travel reconciliation, supply requests, leave requests, monthly in-service);
- Outreach activities with schools and community partners to aid in the growth of the practice, as needed;
- Attend on-site/off-site community engagement activities, clinic events, and/or training as needed;
- Positively contribute to staff morale and corporate culture;
- Interview clients, their families, law enforcement officials, schools, and health and social welfare institutions to obtain client background information;
- Identify the psychosocial, economic, and physical needs of clients;
- Develop individual treatment plans; and assesses and reassesses clients’ support systems and available community resources, schools, and facilities;
- Counsel clients and their families and coordinates placements of clients with community facilities and schools;
- Serve as liaison between clients and their families, state and judicial systems, and facility or treatment providers;
- Facilitate individual contact and group education sessions with patients;
- Fulfill clinical documentation requirements in order to accurately evaluate the client’s progress in treatment and ongoing client and family educational needs;
- Participate in budget planning, unit administration, and program development and evaluation;
- Participate in organizational workgroups;
- Provide discharge planning;
- May submit evaluations and reports concerning staff recommendations, social work coverage in the facility, in-service training needs, budgetary matters, and social work program status;
- Perform other duties as assigned to support HOPE Clinic’s Mission, Vision, and Values.
QUALIFICATION REQUIREMENTS:
- Knowledge of community resources and healthcare systems;
- Excellent communication and interpersonal skills;
- Ability to work independently and as part of a team;
- Strong organizational and time management skills;
- Competent in using Microsoft Word, Excel, Power Point, Google Suite, Zoom, and other software applications.
EDUCATION and/or EXPERIENCE:
- Master's degree in social work, psychology, or related field;
- Experience working with individuals and families in need of behavioral health, maternal health, and children's services;
- 2+ years of prior experience working in the behavioral health field or closely related field.
OTHER SKILLS and ABILITIES:
- Bilingual (Vietnamese, Chinese, Arabic, and/or Spanish with English) is required;
- Above average skills in language ability as well as public speaking and writing;
- Must have good transportation and a valid Texas Driver’s license.
