Greater Nashua Mental Health is looking for MAT Nurse Coordinator to join our team! The MAT Nurse Coordinator will coordinate the nursing component of the Medication Assisted Treatment Program, assuring that agency, departmental and professional treatment goals are met. Provides nursing care under the direction of the medical staff in accordance with evidence-based practice. Provides nursing support to other agency programs.
Duties & Responsibilities:
- Provides direct nursing care for clients receiving medication assisted treatment for opioid use disorder and alcohol use disorder, including screening and telephone triage, performing intakes, performing urine toxicology screens, conducting point of care screening for HIV and Hepatitis, supervising buprenorphine induction and providing counseling on medication assisted therapies for substance use disorders. Administers prescribed injectable medications including Sublocade, Brixadi and Vivitrol.
- Manages care coordination by scheduling clients, ensuring lab work is completed, maintaining documentation of services and outreach, coordinating prescriptions with pharmacies and ensuring proper and secure storage of medications.
- Works under guidance of MAT prescribers and follows all state and Federal guidelines.
- Provides nursing care to GNMH clients on other clinical teams as directed by Manager of Nursing and Wellness which includes medication management and education, injections of long-acting medications, nursing assessments, phone triage and assisting medical providers with client refill requests and prior authorizations for medication.
- Perform Case Management duties such as phone triage/crisis interventions and other duties as assigned. Assess consumers in crisis situations for risk of suicide or homicide and coordinates treatment teams to facilitate access to the emergency room when needed.
- Consults appropriately with medical staff, other GNMH providers, family, significant others and community providers. Requests signed releases from client for providers outside of GNMH and for natural supports.
- Assure that clients are treated in accordance with agency client rights, policies and procedures.
- Maintain client records in a timely and accurate manner in keeping with GNMH policy.
- Is familiar with the agency policies and procedures and has a knowledge of other services provided within the agency and community.
- Participate in training/staff development activities consistent with in-service training schedules and staff development plans.
- Perform other duties consistent with employee’s education, training or experience as assigned.
Qualifications:
- Bachelor’s degree in Nursing
- NH Registered Nursing licensure
- CPR Certification
- Demonstrated skill in communication, interpersonal relations, and crisis intervention.
- Is of good professional character.
- Demonstrates knowledge of and skill in the use of relevant computer-based programs such as Microsoft Word, Excel, Outlook, Teams. Demonstrates knowledge of and skill in the use of electronic health records.
- Demonstration of leadership capability and skill in working effectively with colleagues.
- A minimum of (1) year(s) of experience in working with the target client population or particular administrative field, or any equivalent combination of education and experience. Experience working in substance misuse treatment strongly preferred.
- Ability to establish and maintain an effective working relationship with others, to express ideas clearly and concisely in written and oral form, to exercise initiative and work independently, and to exercise sound judgement analyzing situations and recommending appropriate courses of action.
