Description

Mental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development, and the delivery of direct services. Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming an AOT Care Coordinator.


The AOT Care Coordinator functions as a member of an interdisciplinary team to provide care coordination to a caseload of severely mentally ill adult’s court ordered to participate with a medication and treatment regimen. The AOT Care Coordinator supports the client, engages with community agencies/health care providers and others on the members’ behalf to ensure access to services needed to increase wellness, self-management and reduce emergency room visits and/or hospitalizations. The AOT Care Coordinator provides support to the team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions and resources to maintain focus on outcomes and best practices.


Essential Duties & Responsibilities

  • Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals and resources. 
  • Maintains electronic case files and keeps the Supervisor/Director informed and up-to-date on client issues.
  • Participates in the development/documentation/review and update of client centered comprehensive, integrated, interdisciplinary care plan in consultation with the Care Manager and other team members to ensure focus on desired outcomes.
  • Maintains effective communications with clients, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources and other agency staff on behalf of clients.
  • Provides weekly documentation to the County on the progress of AOT clients. 
  • Maintains documents, records, statistics, and other related reports in an organized, timely and accurate manner.
  • Coordinates care planning with other providers of services/resources to ensure goal directed collaborative care, including care transitions.
  • Works as a part of a Care Coordination team; attends and participates in team meetings to provide input/feedback around psychosocial conditions to review client status, update plans and goals and review outcomes to further program goals.
  • Participates in staff meeting as well as bi-weekly supervision meeting with Supervisor/Director.
  • Engages clients to participate in full treatment as listed in AOT order.
  • Provides telephonic as well as face to face outreach, engagement and service planning in the field.
  • Acts as a linkage to community services including medical, behavioral, residential, substance abuse, entitlement and any other needed services per interdisciplinary care plan.
  • Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers/resources as needed.
  • Provides crisis intervention and follow-up.
  • Other duties as assigned.
Requirements


Education & Experience

  • Master’s Degree with One year of Experience with providing direct services to people with Serious Mental Health Illness, Developmental Disabilities, alcoholism or substance abuse
  • Master’s Degree in: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy.
  • Bachelor level candidates, Two (2) years of Experience with providing direct services to people with Serious Mental Health Illness, Developmental Disabilities, alcoholism or substance abuse
  • Degree in: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy.
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and Two years of experience
  • Bachelor’s Degree or higher in any field with either 3 years of experience or 2 years of experience as a Health Home Care Manager servicing SMI or SED.

Other Qualifications

  • Bilingual in Spanish and English required
  • Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model.
  • Knowledge of Medicaid, Social Security and other entitlements preferred.
  • Excellent interpersonal skills required.
  • Must have the ability and willingness to regularly travel to various locations in Nassau County, using agency vehicle when transporting consumers.
  • Valid NYS Driver’s License; must take defensive driving course through agency or external provider
  • Working knowledge of computer software (Microsoft Word, Outlook) and electronic health records systems
  • Strong verbal and written communication skills required as well as detailed oriented, organized and ability to handle multiple projects at the same time.
