  • Salary Range: $85,000 - $105,000 annually (with flexibility up to $120,000 for highly qualified candidates)

Position Summary:   

The Practice Manager for the ElderCare Program at Snowline Health is responsible for overseeing the day-to-day operations of the ElderCare Program. This role ensures efficient management of clinical and administrative staff, coordinates patient care services, optimizes operational workflows, and drives program excellence to deliver high-quality, compassionate care to elderly patients.

The Practice Manager works collaboratively with clinical teams, support staff, and leadership to ensure compliance with regulatory requirements, alignment with Snowline Health's mission, and the achievement of strategic goals.


Key Highlights of Essential Functions:

  

Operational Management

  • Manage daily operations of the ElderCare Program by combining experience and contemporary leadership practices with Snowline Health Eldercare Mission in achieving excellence ensuring efficiency, effectiveness, and adherence to established policies and procedures for Home-Based Primary Care (HBPC)
  • Accountable for providing strategic vision and operational planning when developing and implementing workflows and procedures for ElderCare start-up and ongoing programmatic development 

Staff Leadership and Development

  • Supervise, train, and mentor administrative and clinical support staff
  • Oversee staffing schedules to ensure adequate coverage while managing labor costs
  • Work directly with ElderCare Medical Director and Snowline Health Advisory Committee to develop and provide ethical, medically necessary care structured care plans for service population

Patient Care Coordination

  • Ensure seamless coordination of patient services, including scheduling, referrals, and follow-up care. Will set up these initial processes with SLH support

Regulatory Compliance and Quality Assurance

  • Maintain compliance with all regulatory requirements, including HIPAA and Medicare guidelines.

Financial Oversight

  • Assist in budget preparation and management for the ElderCare Program
  • Monitor expenses and revenue, identifying opportunities for cost containment and revenue enhancement.
Requirements

Your Experience and Education

  • Minimum five years in a healthcare setting preferred
  • Minimum three years of work experience in a medical practice leadership position or healthcare agency administrative role
  • Licensed nurse or practitioner preferred but not required
  • Knowledge of medical terminology or willingness to complete recommended education within set timeframes
  • Excellent communication skills, demonstrate advanced skills in assisting individuals and families in problem-solving and utilizing community resources
  • Excellent observation, problem-solving, verbal and written communication skills
  • Demonstrates computer skills with extensive experience using Microsoft Office, Word, and Excel required with a basic functional understanding of medical EMRs
  • Demonstrates the ability to efficiently prioritize tasks and manage multiple objectives and team members. 
  • Delegate duties to other team members, while effectively carrying out job duties 
  • Understands either through experience or professional proximity, the stressors of start-up healthcare initiatives; and willingness to lead a team through program development from day one.
  • Valid CA driver’s license with a reliable vehicle that is insured in accordance with state requirements
