CareLink is a non-profit, mission-based 501c3 organization that was founded in 1997 by non-profit post-acute and community-based organizations specializing in the care of older adults and adults with disabilities. These organizations include long-term care, housing, hospice, home care, assisted living and adult-day health. In addition, CareLink is a direct provider of a range of health and wellness services including rehabilitation (PT/OT/SLP), mobile dental, audiology, podiatry, psychiatry, brain health and wellness services. We keep individuals doing the things that they want to do, independently, in the place they call home.
This part-time (10-15 hours per week) position would provide care within the state of Rhode Island.
The Wellness clinician works collaboratively with the Wellness Program Manager to implement a variety of programming throughout the state including evidence-based fall prevention programs, social isolation/mental wellness groups and exercise programs focused on improving the lives of older adults.
Essential functions:
- Provide individual support or counseling in the specialty of holistic wellness including but not limited to mental, physical (i.e. nutrition), social, spiritual, and/or environmental.
- Track attendance, participation, or performance data related to wellness events.
- Communicate needs for maintenance of fitness equipment or facilities to the Program Manager.
- Lead the fall prevention program within our facilities and gather participation interest from our team.
- Must be willing to provide training in, and serve as an instructor of, evidence-based wellness programming such as TJQMBB, MOB, SAIL, and/or fitness classes. (Type of programming and percentage of overall time will be determined based on company needs.)
- Continually evaluate program offerings and recommend new programs or services to promote holistic wellness, ensuring a diversity of class offerings.
- Support the Program Manager in organizing and overseeing health screenings, wellness events/programs, or classes; wellness-related schedules, records, and reports.
- Additional projects as requested by the Program Manager
- Fulfills all job functions within scope of practice.
- Maintaining professional knowledge in order to provide clients with evidence-based standards of practice.

Education/Experience Required:
- Valid Certified Occupational Therapy Assistant or Physical Therapy Assistant license in the state of Rhode Island
- Experience working with the geriatric and/or adult IDD populations
- Must possess a valid and current driver license and auto insurance.
- Willing to obtain or possesses current BLS and/or CPR certification from the American Heart Association or American Red Cross.
- 15+ years clinical experience and extensive life experience strongly preferred.
- Experience with TJQMBB, MOB, SAIL, and/or fitness classes strongly preferred.
