Description
Hospice of the Piedmont is a life-changing, mission-driven organization offering a team-based work environment with a supportive management team. We are seeking a full-time Hospice Aide who provides personal care and/or homemaker services supporting the patients and caregivers in a manner that is meaningful and reflects the Hospice philosophy and concepts of care.
Responsibilities/General Duties (Including but not limited to):
- Assists patients with personal hygiene, including shower, tub or bed baths and mouth, skin and hair care – all activities of daily living that are required.
- Assists patients in the use of toilet facilities, including bedpans, incontinent care etc.
- Assists patients in and out of bed, including the use of mechanical lifting equipment, when applicable
- Assists patients with walking, including the use of walkers and wheelchairs, when applicable
- Assists patients with self-administration of medications
- Assists with simple meal preparation and feeding, when required
- Measures and records fluid intake and output
- Takes and records blood pressure, pulse and respiration when required
- Maintains and uses EMR according to agency guidelines
- Records and reports changes to the nurse supervisor in the patient’s physical condition, behavior or appearance
- Documents services delivered in accordance with Hospice medical records policies and procedures, and submit documentation in a timely fashion i.e.: weekly
- May require light housekeeping duties in patient area
- Assures personal care provided as is ordered by the plan of care and/or nurse’s instruction and in adherence to Hospice’s policies related to respect, privacy, confidentiality and personal property
Typical Physical Demands/Working Conditions:
- The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
- While performing the duties of the job, the employee is regularly required to use hands to handle or feel objects, tools or controls, reach with hands and arms; stoop, kneel, crouch or crawl; and talk to hear. The employee frequently is required to stand, sit and climb or balance. The employee is occasionally required to walk, taste or smell.
- The employee must occasionally lift and/or move up to 75-100 pounds and push up to 200 pounds in a wheelchair. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
- Incumbent must be able to communicate by way of telephone. Keyboarding is required. Regular use of computer screen is required to complete tasks. Ability to drive and to negotiate steps.
- Noise level is low. Physical and emotional health necessary for the accomplishment of position duties.
Requirements
Qualifications:
- High School Diploma or GED
- Registered by the Virginia Department of Health Professions as a Certified Nurse’s Aide
- One year relevant experience required
- Hospice experience preferred
Other:
- Driver’s license, personal automobile and current automobile liability insurance
- Frequent travel may be required
