Job Summary

In collaboration with the Director of Volunteer Leadership, Mentoring and Food Insecurity, the Food Pantry Operations Manager, and the Senior Case Manager for Food Insecurity, the Case Manager will coordinate services and follow-up efforts related to addressing food insecurity.


Case Manager Essential Responsibilities:

  • Weekly visits to each pantry location at minimum, may require more depending on project/time of year.
  • Identify and maintain client dietary restrictions (i.e. low sodium, food for children, kosher) via Electronic Health Records and/or Excel.
  • Process all pantry applications and annual requalification.
  • Independently refer for and coordinate the services needed internally and externally.
  • Advocate to meet client’s needs; consult and collaborate with other professionals and service providers to assure comprehensive care.
  • Ability to be on site to meet with clients needing to access the pantry as needed.
  • Visit clients in their home setting as needed.
  • Follow up pertaining to delivery concerns with clients (not home, no answer, etc.) and all other case management related concerns.
Requirements
  • Master’s in social work or equivalent required/or Bachelor’s Degree with minimum of 4 years’ experience in social work or related field.
  • Experience with geriatric and special populations preferred. 
  • Knowledge of MS Office Suite (Word, Excel) and familiarity with Electronic Health Records Systems.
  • A valid Florida driver’s license is required and must have a good driving record. 
  • Proof of automobile insurance.
  • Strong communication and interpersonal skills, flexibility, and ability to function independently.  
  • Possess an understanding of and compatibility with Jewish values and traditions.
