Duties and Responsibilities 

  • Provide in-person, telephone, and community CHW services to assigned PIHC patients who are enrolled in the HOPWA HINT Housing Program. Service delivery may be via onsite at a PIHC center or in the community.
  •  Establish trusting relationships with patients who are affected by HIV and provide general support and encouragement.
  • Provide culturally appropriate health education and information. 
  • Educate patients about treatment and the appropriate use of PIHC services.
  • Provide goal setting with and supportive services for patients, utilizing basic motivational interviewing. 
  • Help assigned patients schedule and attend appointments.
  • Provide referrals and help link patients to community-based services that will help them stay healthy (e.g. housing, food, mental health, substance use, legal services).
  • Work collaboratively with and be an active participant of assigned PIHC Care Team and attend Care Team meetings as scheduled.
  •  Focus on patients who have recently entered/returned to care at PIHC.
  • Complete an annual EMA Screening Tool for assigned patients.
  • Assist patients in completing applications and registration forms.
  • Attend regular PIHC staff meetings, trainings and other meetings as requested.
  • Attend weekly team meetings.
  • Attends and participates in regular HUD, HOPWA and associated conference calls for the HOPWA HINT program as needed.
  • Manage assigned caseload of patients, to include maintaining a patient spreadsheet with discrete data points.
  • Promote and maintain collaborative relationships with community partners.
  • Compliance with PIHC’s policies and procedures.
  • Learn the use of the agency’s Electronic Medical Record (EMR) system and maintain timely and accurate documentation of all patient interactions.
  • Participate in quality assurance reviews as assigned.
  • Ensures that program deliverables are met or exceeded during the project period. 
  • Ensure that services provided to patients adhere to the programmatic guidelines, goals, and objectives established by Federal, State and local funders.
  • Compliance with PIHC’s policies and procedures.
Requirements

Minimum Qualifications

  • Possess a High School Diploma or G.E.D.
  • Able and willing to work flexible hours, including evenings and weekends as assigned.
  • Possess interest or experience in working with patients who have been difficult to engage and/or demonstrated a challenge with staying retained in health care services.
  • Possess basic computer skills.
  • Able to drive and holds a valid GA Driver’s license.
