Position: Occupational Therapist (OT) - Home Health
Occupational Therapist (OT) - Home Health
Employment Status: Part-Time or Per Diem
Location: Caseload based on your location preference.
(OPO Health is licensed in 15+ counties.)
Essential Functions and Responsibilities
Patient Care:
- Evaluate patients’ functional status and assess their home environments for potential hazards or barriers to independent living.
- Identify equipment needs and participate in developing the total plan of care.
- Develop individualized treatment programs, establish functional goals, and communicate plans of care to the referring physician and other healthcare professionals.
- Teach new skills or retrain patients in daily activities affected by illness or injury, following organizational policies.
- Instruct patients in the proper use of adaptive equipment to enhance functional abilities.
- Maintain accurate clinical records, including clinical notes and reports, and promptly inform the physician of any changes in patients’ conditions.
Collaboration and Coordination:
- Ensure open communication with all personnel involved in the patient’s care to provide coordinated and efficient services.
- Identify and refer patients and families/caregivers to other home health services as needed.
- Provide training to patients’ families, caregivers, and other healthcare personnel regarding treatment regimens when appropriate.
Supervision:
- Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with organizational policies and state regulations.
Requirements
Qualifications and Skills:
- Active and unencumbered California Occupational Therapist (OT) license.
- Valid California driver’s license and proof of auto insurance.
- Minimum of one (1+) year of relevant experience.
- Current CPR certification.
