Description

Springfield Hospital is looking for a compassionate and motivated social worker or nurse to join a small but integral part of the patient management team as a case manager. A strong candidate for this position is one who is empathetic, a strong communicator and active listener, and above all else values a patient’s moral independence. 


This position requires autonomy, but is well supported, requires a strong understanding of the resources in the community but has the opportunity for mentorship, and the desire to improve a patient’s well-being. The Social Worker will:

  • Maintain a good working relationship with other members of the healthcare team. 
  • Identify and assesses patient's psychosocial needs and coordinates with the interdisciplinary team to meet those needs. 
  • Provide follow-up for patients to ensure continuity of care. 
  • Serve as a liaison between hospital and community agencies. 
  • Teach and provide education about Social Services to Staff. 
  • Act as an advocate for patients and their families.
Requirements
  • Bachelor’s degree in Social Work, Psychology, Sociology or Social Services or Associate degree in Nursing 
  • Masters degree in Social Work or Bachelor’s degree in Nursing (preferred)
  • Prior experience with Electronic Medical Record
  • Hospital and/or nursing home affiliation (preferred)
  • Basic Life Support (BLS) for Healthcare Providers
  • Current Vermont LICSW licensure in good standing with no restrictions or stipulations OR Current Vermont RN licensure in good standing with no restrictions or stipulations or multistate compact licensure in good standing with no restrictions or stipulations.
  • Knowledge of medical terminology, healthcare systems, and community resources is essential.
  • Excellent communication and interpersonal skills to collaborate effectively with patients, families, and healthcare professionals.
  • Proficient in Microsoft Office Suite (Excel, Word, Outlook, etc.).
  • Knowledge of insurance processes.
