Our clinical team is seeking a full-time, 32 hr./weekly REFERRAL NURSE or CMA at our Division Street Family Clinic.
Schedule: M-T-W-F, 8:00 am - 5:00 pm

We serve a population of approximately 15,000 and provide various clinical and community outreach services at six full time clinics.

Job Summary
Patients are referred to a specialist when the patient’s needs are beyond the scope of practice for a the provider. The Referral Nurse appropriately and efficiently gathers needed information from external specialty services for established patients and internal referrals. The Referral Nurse works closely with Providers and Site Manager to evaluate patient satisfaction and outcomes of care and to provide coordination of services for patients within an integrated health care system.
SKILLS:
- Demonstrates effective written and verbal communication skills
- Concern for client’s dignity
- EMR (EPIC) experience preferred
