JOB SUMMARY

The Speech Language Pathologist (SLP) provides intermittent speech therapy to patients in their place of residence, based on physician orders and a personalized plan of care. The SLP works under the direction of the Director of Clinical Services or Clinical Manager to improve patient communication skills and support their quality of life.

QUALIFICATIONS

  • Master’s or Doctoral degree in Speech Language Pathology.
  • Licensed as a Speech Language Pathologist in the state of practice.
  • Completion of 350 clock hours of supervised clinical practicum, nine months of full-time supervised SLP experience, and a national examination (if applicable).
  • Two (2) years of experience in speech-language pathology preferred.
Requirements

RESPONSIBILITIES

  • Enhance patient communication and maximize functional abilities within each patient’s capabilities.
  • Administer speech therapy services as ordered by the physician.
  • Conduct initial patient evaluations, create a treatment plan, and submit it to the physician for approval.
  • Perform all skilled procedures and consult with physicians to modify treatment when needed.
  • Document and report patient progress to physicians and prepare accurate progress notes for each visit.
  • Instruct patients and family members on home-based therapy programs.
  • Participate in interdisciplinary patient care planning.
  • Supervise assigned personnel, including Home Health Aides, with supervisory visits every 14 days as required.
  • Assist physicians in evaluating patients’ functional levels and adjust the plan of care as necessary.
  • Complete OASIS assessments, reassessments, transfers, resumptions, and discharges within agency-defined timeframes.
  • Appropriately utilize ICD-10 codes for documentation.
  • Write reevaluation notes as treatment progresses and discharge summaries upon patient completion.
  • Present in-service training to agency staff periodically.
  • Participate actively in Quality Assurance and Performance Improvement (QAPI) activities.
  • Provide advice and consultation to family members and agency personnel about patient care.
  • Attend in-service programs to maintain up-to-date knowledge in the field.

WORKING ENVIRONMENT

Primarily works indoors in office settings and patient homes, with required travel to and from patient locations.

  • Supervised by: Director of Clinical Services/Clinical Manager
  • Supervises: Home Health Aide as assigned

RISK EXPOSURE

High risk due to direct patient care responsibilities in home health settings.

LIFTING REQUIREMENTS

The role involves necessary physical activities, including extended periods of standing, moderate lifting, and regular bending and movement.
