Position Summary:
Under the direction of the Director of Behavioral Health Services, the case manager will provide case management and supportive services to guests in the residential treatment program who have co-occurring disorders and are experiencing homelessness

  • Develops individualized case plans including identification of existing barriers to self-sufficiency, by setting housing and income goals.
  • Makes referrals to appropriate providers and resources.
  • Coordinates and advocates with other service providers are involved in the client’s care as part of a team-based approach.
  • Confers with case managers and program supervisors on all cases.
  • Participates in case staffing.
  • Meets with clients on a regular basis to discuss, assess, direct, and assist in the removal of barriers to self-sufficiency.
  • Completes all required paperwork and reports accurately and in a timely manner.
  • Maintains accurate and ongoing documentation in client files and on recordkeeping forms and reports adhering to contract requirements, department, and agency policies and procedures.
  • Completes with all confidentiality standards.
  • Documents all client encounters utilizing the homeless management information system (HMIS) and other client information systems, as necessary.
  • Participates in interagency and community meetings to initiate and promote placement of clients.
  • Responds to client emergency needs and conducts follow-up services.
  • Devises and implements placement and follow-up procedures.
  • Works effectively and professionally with staff from other departments and community partners.
  • Assists with special projects as needed and attends staff meetings.
  • Other duties as assigned.

Physical Requirements:  While performing job duties, an employee is required to stand, walk, and use hands/fingers. May occasionally lift and move up to 25 lbs., clean (sit, bend/twist neck, bend/twist waist, squat, climb, balance, stoop, crouch, reach, handle, fingering, kneel, crawl, reach above/below shoulder height, repetitive movements, wipe, sweep, dust, mop). Specific vision abilities required by the position include close vision, color vision.


Work Environment: The noise level of the work environment can range from low to moderate.


Work Schedule: 

Non-Remote Position

  •  Full-time: Monday through Friday, 8:00 AM to 4:30PM
  • Ability to work a flexible work schedule (i.e., Evenings, weekends, holidays, and on-call), as needed. 
Requirements
  • Ability to understand and practice the mission of Camillus House.
  • Education: Bachelor’s degree in social work or related field. Any combination of education and job experience will be considered.
  • Language: Proficiency in oral/written English. Spanish preferred; additional languages a plus (i.e., Creole).
  • Skills: Proficiency in computer (Word, Excel, other data collecting programs).
  • Strong organizational and managerial skills.
  • Ability to work independently.
  • Successful completion of Toxicology test, Level II Screening, and OIG Reference Check.  
  • Valid FL Driver License with clean driving record.
  • Culturally sensitive to the social, psychological, and healthcare needs of the vulnerable populations serviced.
  • Strong organizational, interpersonal, and communication skills to meet deadlines and handle multiple tasks.
  • Demonstrates a high level of productivity while remaining calm and effective under pressure.
  • A self-starter with demonstrated ability to work independently, as well as with multidisciplinary teams and co-workers.
  • Ability to maintain a high level of poise and professionalism in all circumstances.
