Scope of Position: Under the direction of the Program Director, and within the guidelines of all applicable laws, rules, regulations and the Agency code of ethics and policies and procedures, The Mental Health Clinician works collaboratively with a multi-disciplinary team to provide services aligned with the program model and actively participates in daily team meetings to review and plan client care. The clinician is responsible for delivering the required number of services each month, 80% of which are provided in the community. This role involves completing all necessary documentation in a timely and professional manner while working closely with clients, their families, and community partners to support the achievement of treatment and recovery goals.


Essential Position Functions:

  • Provide individual and group counseling, substance use treatment, and evidence-based therapies (e.g., CBT, harm reduction, motivational interviewing, trauma-informed care).
  • Participate in team meetings to develop, review, and adjust treatment plans.
  • Complete documentation on time and in compliance with regulatory standards.
  • Manage daily needs of program participants, including stipends, requests, job functions, and disciplinary measures.
  • Ensure groups and activities are carried out as scheduled.
  • Attend case conferences and collaborate with multi-disciplinary teams to deliver person-centered care.
  • Coordinate care with hospitals, treatment facilities, probation, parole, and other agencies.
  • Conduct assessments (psychosocial, substance use, violence risk, etc.) and develop individualized service, crisis, and relapse prevention plans.
  • Identify or develop group curricula to support recovery in substance use, behavioral health, and physical health.
  • Provide referrals for housing, family support, entitlements, employment, and legal advocacy.
  • Assist participants with housing, including completing forms and supporting move-in processes.
  • Offer 24-hour crisis intervention on-call services and conduct immediate needs assessments.
  • Engage in persistent, strength-based outreach to ensure ongoing participant involvement.
  • Perform other duties as needed to ensure program success and participant outcomes.
Requirements
  • Must have or be eligible for a NY State ID (non-driver’s license or driver’s license).
  • Education: MSW, LMSW, LMHC, or LMHC-LP.
  • Licensed and unlicensed Mental Health Clinicians are encouraged to apply.
  • At least two years of experience working with adults diagnosed with serious mental health conditions.
  • Familiarity with electronic health records (specifically AWARDS by Footholds) is preferred.
  • Experience working with homeless or precariously housed populations is preferred, but not required.
  •  Schedule: Monday – Friday, 9:00 AM – 5:00 PM. 
  • Spanish-speaking skills are highly desirable.
