Job Posting: Licensed Practical/Vocational Nurse (LPN/LVN)
Job Summary:
We are seeking a dedicated Licensed Practical/Vocational Nurse (LPN/LVN) to join our home health care team. The LPN/LVN will administer skilled nursing care to patients on an intermittent basis in their place of residence, following physician orders and the care plan under the supervision of a Registered Nurse. This position requires providing services in alignment with Agency policies, ensuring high-quality care and patient satisfaction.
Responsibilities:
- Understand and adhere to established policies and procedures.
- Implement nursing care plans for each patient, ensuring compliance with physician orders.
- Provide nursing services, treatments, and diagnostic and preventive procedures as assigned.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Observe signs and symptoms and report changes in patient conditions, reactions to treatments, and medications to the physician and Registered Nurse.
- Teach and counsel patients and their families/significant others regarding nursing care and self-care techniques.
- Evaluate nursing services' effectiveness in collaboration with the Registered Nurse, assessing the impact on patients and families.
- Maintain accurate and complete records of patient observations, treatments, and care.
- Participate in medical record audits as assigned.
- Attend staff meetings, patient care conferences, and inservice training sessions as scheduled.
- Take on-call duty during nights, weekends, and holidays, as assigned.
- Submit any changes in schedule to the Director of Clinical Services/Clinical Manager on a daily basis.
- Participate in patient care conferences, identifying needs for specialized care, such as physical therapy or speech language pathology.
- Prepare and maintain clinical and progress notes for each patient visit.
- Assist the physician and Registered Nurse in performing specialized procedures.
- Prepare equipment and materials for treatments, ensuring proper sterilization and aseptic technique.
- Assist patients in learning self-care techniques as part of their recovery or ongoing health management.
Working Environment:
- Primarily works indoors in patient homes and the Agency office, with travel to/from patient residences.

Lifting Requirements:
- Ability to perform physical tasks, including heavy lifting, bending, and standing for extended periods.
- Ability to engage in physical activity as necessary during patient care.
Requirements
Qualifications:
- Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) of practice.
- A minimum of one (1) year of nursing experience, preferred.
- Acceptance of the Agency’s philosophy and goals.
- Ability to exercise initiative and independent judgment in patient care.
