Scope of Position: Residential Scattered-Site Case Managers support formerly homeless adults living with serious mental illness and/or substance use disorders. In this impactful role, you will provide comprehensive case management services, assisting clients in accessing housing, healthcare, benefits, and community resources, while fostering a supportive, trauma-informed environment. Your work will be crucial in empowering individuals to develop the skills necessary to maintain housing and achieve long-term stability. By building strong, trusting relationships and offering personalized support, you will guide clients on their path to independence, improving their quality of life and fostering hope for a brighter future. 

 

Essential Position Functions:

  • Provide case management and interventions for high-risk clients, including conducting assessments and facilitating referrals. These services are primarily delivered in clients' apartments and community settings.
  • Identify clients' strengths, needs, abilities, and goals, and create related Support Plans in the electronic client records system.
  • Use Motivational Interviewing techniques to help clients develop skills needed to maintain housing. Support individuals in developing natural supports and include them in Support Plans and client-related meetings.
  • Ensure the timely completion of required documentation for every client contact, following agency policy and regulatory guidelines for the level of care.
  • Co-facilitate groups, as requested, in areas such as wellness, leadership, housing stability, employment and education, relationships, and healthy decision-making.
  • Provide case management services, including assistance with housing applications, referrals to medical and mental health care, maintaining active benefits, escorting clients, ensuring compliance with service providers, and coordinating care with external providers.
  • Attend case conferences with the High-Risk Coordinator to develop interventions.
  • Attend staff meetings, supervision sessions, and trainings as required.
  • Assist in preparing the program for audits by ensuring compliance with requirements, managing admissions and discharges, maintaining accurate census records, overseeing documentation, and handling incident reporting.
  • Collaborate with other departments, external agencies, and treatment providers.
  • Work with consumers/residents, families, community tenants, and staff in a compassionate and respectful manner, with an understanding of cultural differences.
  • Conduct apartment inspections on multiple floors, using stairs, and provide escorts for clients throughout the community via public transportation.
  • Immediately report serious incidents, serious incident allegations, or sensitive situations to supervisors, and complete incident reports in accordance with Bridge policy.
  • Account for consumers, conduct diligent searches, and file missing person reports on clients not accounted for, following policy and procedure.
  • Provide crisis intervention as necessary, using de-escalation techniques.
  • Perform any other duties as assigned, including potentially supporting other programs within the agency.
Requirements
  • Bachelor's degree required.
  • Experience working with mental health issues, crisis intervention, and managing trauma-triggered behavior.
  • Excellent time management and organizational skills.
  • Ability to work effectively as a team player, remaining stress-resistant, flexible, creative, and open to feedback.
  • Willingness to perform CPR, First Aid, AED, and Naloxone administration.
  • Preferred knowledge of the AWARDS system.
  • Bilingual in Spanish is a plus.
  • Experience working with veterans or direct experience with veterans is preferred.
