Scope of Position: The Engagement and Enrollment Specialist plays a vital role in connecting eligible individuals to essential Care Coordination services under the guidance of the Supervisor and Program Director. This role involves assisting both Medicaid and Non-Medicaid recipients who are managing medical, mental health, and substance use needs, ensuring they receive the care and support they require. The Specialist will engage with members through phone calls, written communication, and face-to-face interactions to facilitate their enrollment and retention in Health Home Care Coordination. Working closely with the Care Coordination team, the Specialist will help identify and reach out to potential members, providing them with valuable information about community resources to support their wellness goals. This position is grounded in a Trauma-Informed, Harm Reduction, and Person-Centered approach, making a meaningful impact on the lives of individuals in need. 

 

Essential Job Functions:

  • Engage and enroll Health Home eligible individuals through telephonic, written, and face-to-face interactions to facilitate enrollment and retention.
  • Maintain accurate clinical documentation in Electronic Health/Medical Record system(s), ensuring compliance with HIPAA regulations and confidentiality protocols.
  • Assist Care Coordinators with securing referrals to medical, behavioral health, and social assistance providers through telephonic, written, and face-to-face outreach.
  • Conduct follow-up services including reminder calls, medication management, and wellness checks as needed, while participating in meetings and case conferences within the community.
  • Attend community meetings and case conferences to stay informed and engaged in service delivery.
  • This role requires approximately 75% fieldwork and travel via mass transit between site locations.
  • Perform any other duties as assigned.
Requirements
  • Bachelor’s degree.
  • Minimum of 1 year of job-related experience providing medical, mental health, or substance-focused services to individuals with chronic medical conditions and/or serious behavioral health conditions.
  • Excellent interpersonal, organizational, writing, and computer skills.
  • Ability to engage individuals and effectively diffuse difficult situations.
  • Proficiency in computer use and documentation; familiarity with electronic health records, particularly AWARDS by Footholds, preferred.
  • Willingness to travel by public transportation through the Bronx, Brooklyn, or Manhattan; field-based work required. 
  • Experience in outreach, client engagement, assessment, direct service, treatment, and documentation.
  • Bilingual in Spanish is a plus.
