Schedule: Monday - Friday 9:00am - 5:00pm


Chicago Family Health Center is seeking an Community Health Worker who will work under the Manager of Care Coordination to promote health and decrease disparities in healthcare access. Under the general guidance of the Clinic Manager, the Community Health Worker will work interpedently in the community with specific targeted populations. The Community Health Worker will work collaboratively with all healthcare professionals, social service agencies and various outreach programs to connect patients to resources to improve patient care and outcomes. Serves as an advocate for the health needs of CFHC patients and assists in supporting medical providers and the management team by creating community outreach programs and activities. Oversees outreach programs that are designed to promote, maintain, and improve the health of the patients and their family CFHC serves. 


Responsibilities:

Community Health Responsibilities:

  • Identifies and conveys the purposes and services of a program to CFHC patients or specific populations and the positive impact that the program or services provides
  • Serves as a liaison between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery
  • Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community that CFHC support.
  • Establish positive, supportive relationships with participants and provider feedback for improvement.

Patient Care Responsibilities:

  • Advise patients or community groups on issues related to improving general health, such as diet or exercise as well as assisting with developing health management goals
  • Coordinate transportation for their patients and conduct eligibility determination of services to the uninsured
  • Assisting patients in referrals, follow-ups, and completing applications for service
  • Assist patient in understanding care plans and instructions as well as follow-up with health management/care plans with both patients and providers
  • Document and maintain activities, service plans and updates to patient records with plans, notes, appropriate forms, or related information in a timely manner
  • Educates client on the proper use of the Emergency Room and provides information for alternatives

Requirements

Education and Experience

  • High School Diploma or GED required
  • a minimum of one (1) year to three (3) years of experience related to community outreach or activist organizations and/or health and social services experience 
  • Community Health Worker (CWH) Training/Certification required
  • Evenings and weekends schedule flexibility required
  • Reliable transportation is required.
  • Bilingual ability may or may not be required based on individual department needs.

  • Demonstrated ability to collaborate effectively in a team setting.
  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Experience in working with a diverse population base preferred. 
  • Ability to appropriately handle confrontation and conflict. 
  • Ability to work independently and to seek consultation as necessary.
  • Ability to follow organization and departmental processes and procedures.
  • Ability to work in a high stress/fast paced/high work volume environment.
Salary Description
$17.75 - $25.00
