WHAT YOU'LL DO:
The Clinical Pharmacist is an integral member of the medical team providing safe, effective and timely medication therapy to the patients of Elica Health Centers in the outpatient setting. Elica Health Centers is a Federally Qualified Health Center (FQHC) providing comprehensive primary and preventative health care services to a growing patient population. We are committed to providing excellent service to our patient population. Our rapidly expanding center offers services that include: internal and family medicine, pediatrics, dentistry, care management, and behavioral health. Elica is dedicated to serving low-income, multi-ethnic populations throughout the greater Sacramento area with a special focus on immigrant communities. We are a diverse team passionate about meeting the needs of our community through patient-centered, compassionate, comprehensive, integrated care.
The Clinical Pharmacist will assist the clinical team by conducting medication reviews and providing refills and new prescriptions as needed based on established practice agreements and protocols. The Clinical Pharmacist will consult with both patient and providers as needed regarding medication issues, interactions, side effects, compliance, and other concerns.

WHAT ARE WE LOOKING FOR?
The successful candidate will be willing and able to:
- Facilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards
- Assess the status of the patient’s health problems and determine whether the prescribed medications are optimally meeting the patient’s needs and goals of care.
- Evaluate the appropriateness and effectiveness of the patient’s medications.
- Recognize untreated health problems that could be improved or resolved with appropriate medication therapy.
- Follow the patient’s progress to determine the effects of the patient’s medications on his or her health.
- Consult with the patient’s primary care provider and other health care providers in selecting the medication therapy that best meets the patient’s needs and contributes effectively to the overall therapy goals.
- Advise the patient on how to best take his or her medications.
- Support the health care team’s efforts to educate the patient on other important steps to improve or maintain health, such as exercise, diet, and preventive steps like immunization.
- Refer the patient to their primary care provider or other health professionals to address specific health, wellness, or social services concerns as they arise.
- Provide a consistent process of patient care that ensures the appropriateness, effectiveness, and safety of the patient’s medication use.
- Consult with the patient’s primary care provider and other health care providers to develop and implement a medication plan that can meet the overall goals of patient care established by the health care team.
- Apply specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions, in performing their patient care activities in collaboration with other members of the health care team.
- Call on their clinical experience to solve health problems through the rational use of medications.
- Rely on their professional relationships with patients to tailor their advice to best meet individual patient needs and desires.
- Offer cultural and linguistically appropriate services to a diverse and often socioeconomically challenged population.
- Maintain current knowledge of policies and procedures as they relate to safe work practices.
- Follow all safety procedures and report or resolve unsafe conditions to ensure an injury-free work environment.
- Use appropriate ergonomic measures to reduce risk of work-related injuries.
- Assure adherence to infection control procedures in the clinic, including bloodborne pathogen protocols.
- Attend scheduled provider meetings
The successful candidate has:
- Graduate of an ACPE accredited College of Pharmacy
- Doctor of Pharmacy or other advanced degree highly preferred
- Completion of ASHP accredited PGY-1 Pharmacy Residency
- Completion of PGY-2 Specialty Pharmacy Residency is preferred
- Current Basic Life Support certification.
- Experience with EHR (EPIC experience preferred)
- Maintain physical and mental health status in order to demonstrate professional and ethical competence in providing high quality medical care to Elica clients.
- Interpret, adapt and apply treatment protocols/guidelines and procedures.
- Fluency in Russian, Spanish, Arabic, Farsi, or Dari preferred, though not required.
Additional Requirements
- Must have a current and valid California driver’s license and the ability to provide proof of personal auto insurance on the vehicle driven during working hours.

Physical Requirements and Work Environment
The work environment is characteristic of normal office conditions. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle or lift. The employee is occasionally required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision and ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
