Position Definition
As part of the case management team, the Community Resource Coordinator is responsible for intake and assessment of youth referred through The Harbor Juvenile Assessment Center (HJAC) and the homeless youth that access services at the Nevada Partnership for Homeless Youth’s (NPHY) Drop-In Center. The position will be co-located at NPHY’s Drop-In Center and a HJAC and will involve some travel depending on program needs. As such, this position’s location and hours are subject to change.
Essential Responsibilities
- Complete HJAC intake assessments and follow-up case notes for clients accessing services.
- Complete data entry into client databases; compile and transcribe completed case notes for data entry; ensure accuracy of all data entered for both NPHY and HJAC systems.
- Compile and submit required reports for both NPHY and HJAC in a timely manner.
- Maintain client documents in case files; assist with the creation and organization of new client case files.
- Answer phones and route ‘youth-in-crisis’ calls to appropriate staff; provide and make appropriate community referrals.
- Serve as the homeless youth provider expert and as the NPHY liaison representative at HJAC, and connect youth to NPHY services and other homeless youth agencies when appropriate.
- Present information on NPHY programs and services to potential clients, other HJAC staff, and the community.
- Utilize the Positive Youth Development (PYD) model when working with clients.
- Maintain client records, in the Homeless Management and Information System (HMIS) and NPHY case files, including but not limited to records of expenditures, activities, referrals, contacts, and other client information.
- Participate in staff meetings for both NPHY and HJAC, and multidisciplinary meetings on behalf of client and their family’s wellbeing.
- Other duties as assigned.
- A minimum of a High School diploma or equivalent is required.
- A strong interest in non-profit, social work, or human service field.
- Must be at least 21 years of age and can successfully pass a drug test and criminal background check.
- Ability to complete data entry accurately into various system and reports.
- Able to maintain a high level of professional and ethical standards.
- Knowledge of Office Suite programs (Word, Excel), internet, and email.
- Current and valid Nevada driver’s license, proof of acceptable driving record and the ability to arrive on-time for all scheduled shifts.
- Ability to drive a motor vehicle, walk, lift and carry items up to 40 lbs., push, stoop, kneel, and stand for long periods of time.
Qualifications
- Experience with youth case management is preferred.
- Ability to communicate professionally and effectively both orally and written.
- Knowledge of confidentiality laws and ability to adhere to them.
- Ability to be flexible in work style and schedule; must be available to work nights and weekends.
- Ability to speak, read, and write in Spanish preferred but not required.
