Description
The Case Manager I will:
- Promote patients’ health maintenance and restoration to provide quality, cost-effective outcomes.
- Initiate a discharge plan to meet immediate post hospital needs, as well as long term needs, to optimize health maintenance and prevent readmission.
- Educate patients, family, and staff.
- Collect, analyze, and interpret data to evaluate current processes, and alter practices that don't achieve optimal patient care and customer service.
- Seek learning opportunities to expand current clinical knowledge, as well as cost containment strategies.
Requirements
- Associate Degree in Nursing (ADN) or certificate from an accredited school of nursing
- Bachelor of Science in Nursing (BSN) from an accredited school of nursing. (preferred)
- Two (2) years’ Med/Surge experience
- Hospital or outpatient case management or patient care coordination (preferred)
- Current RN license in the State of Vermont or multi-state compact RN license.
- Case Manager certification within two years of hire
- Basic Life Support (BLS) for Healthcare Providers
- Membership(s) in professional organizations that enhance professional growth..
- Strong critical thinking skills
- Effective written and verbal communication skills.
- Experience with electronic medical record systems.
