The Clinical Support Specialist reports directly to the Regional Director of Operations. Please note this position requires you to have knowledge in Skilled Facilities and will be 100%
PURPOSE
This position is responsible for providing overall support to the field in clinical processes, training and support of the health and well-being of the residents within their designated portfolio while ensuring quality care programs are compliant. Responsibilities include but are not limited to quality assurance and regulatory compliance; assisting in the training and on-boarding of clinical and care coordinator team members. This individual maintains a professional, supportive and positive working relationship with each Community, partners strongly with the Wellness Director, and works as a team with the Regional Director of Operations.
Regional travel is required for this position.
PRINCIPLE DUTIES AND RESPONSIBILITIES
Resident Care
- Provide counsel to Wellness Directors on accurate and timely level of care assessments for residents.
- Ensures proper follow through and documentation for residents with a change in clinical care needs
- Partners with Wellness Directors in portfolio to ensure completion of clinical sections of the Wellness Evaluation & Assessment and PSP at the time of move-in, semiannually and with any change in status
- Acts as Interim Wellness Director when needed
- Review the Quality Assurance audits at each Community. Support the Resident Care team with preparing a detailed plan of correction. To ensure all corrective actions be taken in advance of actual state survey. As measured by achieving materially deficiency free surveys in all assigned communities.
- Reviews the clinical sections of monthly wellness visits and PSP to accurately reflect the resident on a monthly basis
- Ensures communities maintain wellness files according to Phoenix policy, federal and state guidelines
- Reviews field’s monthly MAR/TAR audits to ensure timely completion.
- Demonstrates and knowledgeable of the Six (6) Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse
- Monitors the Medication Rooms, med charts, treatment carts for neatness, cleanliness, availability of medications and expired medications
Strategy
- Develops guidelines, standards, and practices for Resident Care and programming
- Develops ongoing quality improvement programs for resident services
- Participates in development of new systems to result in customer/resident focused environment and company profitability
- Determines and develops risk management programming and communication of issues
- Trends At-Risk reports
Training
- Partners in the delivery and participation in Phoenix Academy training and self-study programs during the required time-frame.
- Completes All Phoenix Academy, job specific and management training in the timeframe designated
- Completes all training required by the state or other regulating authorities including ongoing training per regulation
Compliance/Safety
- Ensures compliance with state assisted living regulations
- Review all state reportable before submission to state regulatory agency
- Ensures compliance with all state and federal regulations (i.e. OSHA nurse practice acts, etc.) and Phoenix standards as it pertains to clinical care
- Understands and practices the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous chemicals immediately
- Oversees community risk management program
- Participate in the development and implementation of a community improvement plan
- Partner with the Executive Director and other community associates to analyze and maintain all resident and associate safety
- Partners with pharmacy consultant to provide optimal pharmaceutical services to residents
- Responsible for infection control programming/training
- Responsible for the safe and efficient distribution / administration of injections, nebulizers, medications and moderate wounds care per state / provincial regulations
- Acts as Lead in an emergency (e.g. elopement, fire, disaster, etc.) according to the chain of authority identified in the community disaster plan
Human Resources
- Interviews Wellness Director candidates for the community
- Assists with active recruitment of open Wellness Director positions
- Participates in Wellness Director development plans and/or action plans
- Fosters Wellness Director growth
- Provides input regarding Wellness staff reassignments or terminations in coordination with Human Resources and Executive Director
EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION
- Must be a Registered Nurse with an Associate or Bachelor’s degree
- Must have a minimum of 2 years department management experience working with elders or disabled individuals
- Must have demonstrated Leadership capabilities
- Multi-department experience strongly recommended
- Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing
