Scope of Position: The Health Intervention Specialist (HIS) plays a vital role in enhancing the capacity of Citywide street outreach teams by providing expert consultation, training, and direct interventions for older adults and individuals with chronic medical conditions who are experiencing homelessness. This position focuses on supporting unsheltered individuals or those recently transitioning from street homelessness to stable housing. The HIS will be an integral part of a multidisciplinary team, ensuring that medically fragile individuals receive the care and resources they need to achieve stability, well-being, and improved quality of life.


Essential Duties and Responsibilities:

  • Provide consultation to outreach staff across NYC, recommending specialized interventions such as illness screenings, relevant assessments, referrals to community-based medical services, psycho-education, and immediate medical care as needed.
  • Facilitate and lead case-specific conferences in various settings, including inpatient and outpatient behavioral/medical health facilities, community-based social service provider sites, participant homes, shelters, and for individuals living unsheltered on the streets.
  • Train staff and deliver direct skill-building sessions to promote optimal health, focusing on preventative strategies to reduce medical risks.
  • Actively participate in multidisciplinary meetings to support coordinated care and collaborative decision-making.
  • Travel throughout Manhattan, Brooklyn, the Bronx, and Queens to provide on-site support and engagement with clients.
Requirements
  • Licensed Registered Nurse (RN) with active licensure.
  • RN degree from an accredited institution.
  • Proven experience working with older adults, including those with complex medical and behavioral health needs.
  • Experience providing care and support for individuals with chronic medical conditions.
  • Experience in working with homeless populations, including those experiencing street homelessness or recently transitioned into housing.
