Job Summary

The Advanced Practice Provider (APP) will provide continuous and comprehensive services necessary for the promotion of whole person care. The APP will provide treatment and primary care to patients in collaboration with and under the supervision of IHC Deputy CMO and/or CMO. The APP works collaboratively with other on-site medical, care team and mental health providers to ensure the delivery of comprehensive and quality patient care.

Required skills and abilities

  • Provide medical treatment and primary care to patients in collaboration with and under the supervision of IHC Deputy CMO and/or CMO.
  • Ensure continuity of care including evaluation, early detection, ongoing management and treatment referral.
  • Obtain patient history to assess status.
  • Perform physical examination, routine pap and pelvic exams, breast exam.
  • Order and review test results.
  • Make judgment regarding health status and treatment needs.
  • Develop care/treatment plan with patient and family.
  • Provide health education and counseling concerning preventive health care, diet, medication, therapeutic treatments, chronic disease, etc.
  • Participates in staff training as required by IHC.
  • Will report direct patient care issues to the Deputy CMO/CMO and any other issue(s) (i.e., administrative) to IHC Practice Manager.
  • Responsible for participating in quality improvement projects/activities and accountable for quality and PCMH.
  • Perform all other duties as directed either formally or informally, verbally or in writing.

Other Requirements

Will assist with daily operations of the Mobile Unit with the Mobile Unit Coordinator including but not limited to:

  • Driving.
  • Assisting with routine cleaning and/or maintenance.
  • Performing pre and post trip checks on the Mobile Unit.
  • Assistance with some of the administrative tasks.
  • Provide health care to patients via Telehealth.
  • Triage and prep patients.
  • May need to perform other tasks typically completed by support staff such as collecting specimens for point of care tests, lab draws, collecting vitals or other clinical duties.
Requirements

Qualifications

  • FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
  • Spanish speaking skills are preferred but not required
  • Hold a valid license in the State of Indiana
  • Current DEA licensure / Prescriptive Authority post-hire
  • Nurse Practitioner certification from either the ANCC / AANP or Physician Assistant certification from NCCPA
  • Certified in Basic Life Support (BLS)
  • 3+ years of clinical experience as an NP/PA in the primary care setting
  • Previous experience working in a Community Health Center is preferred but not required
