Summary:
The Registered Nurse (RN) plays a critical role in executing InterCommunity mission to promote whole person health. Under the supervision and direction of the Clinical Practice Manager, the Registered Nurse is responsible for providing services which consists of an evaluation and assessment of medical and psychiatric status as an integral part of the interdisciplinary team. The Registered Nurse also provides direct patient care in the departments of primary care and psychiatry.
Essential Duties & Responsibilities:
- Provides direct patient care, carrying out orders given by my medical provider, which may include obtaining medical histories, obtaining vital signs, performing office lab testing and phlebotomy/ venipuncture, EKGs, wound care, and which includes but is not limited to medication administration.
- Conducting nursing visits or educational groups as ordered by the medical provider
- Maintains accurate, detailed reports and records.
- Records and reconciles patients' medical information.
- Monitors, records, and reports symptoms or changes in patients' conditions, assess efficacy of prescribed medications.
- Ensure gaps in care for screenings, chronic care tests (such as HgbA1c for diabetes), BMI, vaccinations, are completed in timelines established by best practice.
- Facilitates effective crisis intervention. Perform safety assessments and triage for patients in crisis.
- Consults and coordinates with healthcare team members to assess, plan, implement, or evaluate patient care plans.
- Modifies patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes
- Maintains an active 10- patient panel for Complex Care Management (CCM).
- Performs patient triage of acute conditions in-person or over the phone.
- Refers patients to specialized health resources or community agencies furnishing assistance.
- Performs other duties as required.
*All agency staff are required to attend all mandatory department/agency meetings and trainings.
*All Support Staff are required to show proof of a CPR Certification within 60 days of official start date.
Schedule:
Monday to Friday, 8:30 am to 5:00 pm, no nights, no weekends and most holidays off!
Education &/Or Experience:
- Graduation from an accredited nursing program. Current State of Connecticut RN licensure required. One (1) year medical experience in a public health setting required. Current Connecticut RN License required.
- 2 years of community health, addiction medicine and/or psychiatric nursing preferred.
- Basic Life Support (BLS)/ CPR certification.
- EPIC experience preferred, but not required.
- Bilingual (Spanish Speaking) preferred, but not required.
