General Responsibilities
- Oversee the nursing department in the higher levels of living.
- Ensure staffing schedules are maintained and troubleshoot urgent staffing needs.
- Counsel & evaluate nursing personnel performance using quality assurance standards and established policies & procedures.
- Administer prescribed medications and treatments in accordance with approved nursing techniques.
- Review resident’s records for change in conditions, adverse reactions, incidents and ensure that proper notification has been followed
- Participate in daily rounds.
- Assist with new admission entry process and audits.
- Greet new admissions and troubleshooting any immediate needs.
- Assess resident needs and provide necessary nursing care as per resident's care plan in conjunction with the interdisciplinary team.
- Assure that resident rights are maintained.
- Handle resident, family, or physician complaints/concerns, referring issues to DON as necessary.
- Respect advance directives and assist residents to a dignified death while providing emotional and spiritual support to family and staff.
- Respond to life-threatening situations based upon nursing standards, policies, procedures, and protocols.
- Maintain a clean, safe, homelike atmosphere while respecting each resident's individuality.
- Participate in organizational safety programs and performance improvement activities.
- Attend in-services given by other departments when applicable to this position.
- Demonstrate the ability to work cooperatively with residents, staff, and families.
· Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures.
· Maintain regular and punctual attendance at work and meetings.
· Attends in-service and education programs.
Infection Prevention:
- Champion for the Infection Prevention program
- Oversee the management of outbreaks: contact tracing, isolation needs and reporting
- Maintain line lists and timely reporting to the local health department, IDPH and NHSN
- Develop and implement processes and procedures for immunization and testing for new and existing residents and employees and reporting timely in NHSN
- Ensure compliance with TB screening (Step 1,2 and annual) and Hepatitis immunization/declination.
- Oversee COVID testing, boosters, and flu vaccine program for the higher levels of living.
- Provide infection prevention initiatives for residents and staff.
- Implement and evaluate the community’s antimicrobial stewardship program.
- Ensure compliance with the Exposure Control Plan (OSHA).
- Oversee fit testing for all team members according to OSHA Emergency Temporary Standards.
- Develop policies and processes toward achieving education objectives and monitoring progress.
- Develop nursing department and position competencies related to infection control.
- Facilitate staff education
- Assist in developing and implementing in-service education for HCC and AL POC.
- Participate in New Hire Orientation.
- Ensure necessary orientation and in-service education is available to all employees.
- Participate in annual IDPH survey preparation.
- Participate in the QAPI Committee.
- Develop, communicate, and assure compliance with current, effective departmental safety policies and procedures.
- Report any safety-related issues to the appropriate resource.
- Take part in on-call rotation for HCC and AL/Memory Care.

JOB REQUIREMENTS:
- Associate’s degree in nursing required; Bachelor of Science in Nursing recommended.
- Current Illinois Registered Nurse License.
- Basic Life Support (CPR) certification.
- Minimum one year experience as a charge nurse or nursing supervisor required.
- SNF and LTC preferred.
- Minimum one year’s experience with Point Click Care recommended.
- Experience working with elderly populations.
- Proficient written and oral communication skills.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Understanding of Point Click Care or similar systems.
- Experience in long-term care or assisted living recommended.
- Knowledge of monitoring medication administration.
- Excellent communication and interpersonal skills.
- Ability to work independently and multitask.
- Proficiency with Microsoft Office Suite or related software.
- Strong organizational skills and attention to detail.
- Ability to read, write, and speak English sufficiently to perform job duties and interact with residents, families, and employees.
ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS: The physical activities of this position constantly require effective talking, hearing, sitting, walking, and standing functions. The position also often requires exerting up to 50 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.
OSHA Exposure Category: Category I – Position includes tasks that involve exposure to blood, body fluids, tissues; would be required to perform Category I tasks.
INTELLECTUAL/EMOTIONAL REQUIREMENTS: The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing a variety of duties, often changing from one task to another without loss of efficiency or composure. This position requires the ability to deal with and find resolutions to problems and maintain a productive working relationship with employees and may be interrupted frequently to meet the needs and requests of others. It also requires adaptability to fluctuate between priorities daily while being responsive to needs as they arise.
