Job Summary:


Community Health Workers (CHW) work throughout the Jordan Valley service area to facilitate health equity. CHWs build relationships with patients who access the health center or those within the community who are in need of services. Through that relationship, the Community Health Worker will serve as a liaison between patients and resources. CHWs facilitate access to care, improve quality of life and address the effects of social determinants of health.


Key Responsibilities:

  1. Works directly with the partnering school district to identify needs and set student/patient -centered health and wellness goals.
  2. Builds relationships and interacts professionally with partnering school district staff, (nurses, secretaries, teachers, etc.), community partnerships, healthcare providers, and social service providers.
  3. Responsible for pre-working school-based consent forms and entering information into patient’s charts.
  4. Responsible for coordinating and attending school-based events and child centered clinic events planned by Jordan Valley and the partnering school district.
  5. Schedule and execute school-based appointments such as telehealth appointments, dental exams, well child exams, and vision exams.
  6. Provide referrals and follow-up on referrals following a school-based medical, dental, behavioral, therapy, or vision appointment.
  7. Works directly with a patient’s treatment team and participates in care coordination and/or case management.
  8. Assists patients/students and community members in navigating health and social service systems
  9. Connects patients with resources for basic needs.
  10. Works in a manner that builds individual and community capacity by empowering and educating members of the community.
  11. Provides basic health screenings (i.e Well Child Checks).
  12. Provides culturally appropriate health education and information to advance patient’s health literacy related to their conditions.

Requirements

Required Qualifications:

  • Valid Driver’s License.

Preferred Qualifications:

  • Bachelor’s Degree.
  • 1-year clinical experience or Community Health field.
