Job Title: LVN Medical Case Manager

Status: Full Time and Part Time

Classification: Non-Exempt

Location: Menlo Park

The LVN Medical Case Manger reports to the Program Manager. This is a non-supervisory role responsible for providing specialized care and support to elderly individuals in the Geriatric Health Care for Homeless Veterans program (Geriatric HCHV). This includes coordinating services, managing cases, and ensuring that residents receive the medical, social, and community resources they need to live independently and thrive in a safe and supportive environment. This position will involve working closely with staff, residents, the VA, and healthcare providers.

Job Duties:

  1. Case Management: Assess the needs of participants, create and implement personalized care plans, and monitor progress to ensure all needs are met in accordance with program guidelines. This also includes development of plans addressing Activities of Daily Living (ADLs) and possible crises situations. Assist with obtaining vital documents. Attend and advocate for participants during housing and medical appointments as needed. Assist participants in applying for additional financial or other support that promotes housing stability as needed. Verify participant’s payments for rental and utility bills on a monthly basis.
  2. Nursing Care: Provide basic nursing services including medication monitoring, ADL monitoring, performing health assessments, memory cues, and coordination with healthcare professionals for medical concerns.
  3. Advocacy: Advocate for resident’s needs with service providers, housing, and community resources to ensure that residents have access to necessary healthcare, social services, and housing resources.
  4. Collaboration: Work collaboratively with social workers, the VA liaison, public housing authority, physicians, therapists, and other professionals to ensure comprehensive care for residents. Attend interdisciplinary care team meetings to ensure continuity of care.
  5. Documentation: Maintain accurate and timely case notes and other documentation related to resident care. Ensure all documentation complies with regulatory and organizational standards
  6. Crisis Management: Address and resolve any urgent issues or challenges, such as health emergencies, housing instability, or conflicts among residents, providing swift and effective solutions. Provide basic de-escalation as needed.
  7. Resident Education: Provide education to residents on health-related topics including but not limited to medication management, health maintenance, and available community resources to promote independent living.
  8. Transportation: Provide transportation and attend medical and resource appointments as needed.
  9. Performs other duties as assigned 

Educational Requirements

Associate of Science (preferred)

Bachelor’s degree in related field (preferred)

Qualifications:

Licensed Vocational Nurse in California 

Basic Life Support (BLS) certification

Minimum one year of experience working in a healthcare or case management setting with a focus on geriatrics, housing, or social services.

Knowledge of geriatric care: understanding of common medical conditions in older adults and best practices in elder care, including knowledge of medication management, aging-related health concerns, and social service programs.

Proficient in MS Office applications including Outlook, Excel, SharePoint, and Teams.

Proven ability to work independently while providing consistent, high-quality judgment

DMV record in good standing

Physical Requirements:

Requires the ability to occasionally bend, stoop, reach, lift, and move or carry office supplies and materials weighing 20 pounds or less. Finger dexterity required for computer keyboard operation. 

Work Environment:

Office located in Menlo Park, CA. Team-oriented office environment as well as participant residential environment. This is an in-person role and is not remote eligible.

Offering:

$35 - $41 per hour, depending on qualifications and geographical location.
