Job Title: CNA Caregiver

The CNA (Certified Nursing Assistant) Caregiver reports to the Program Manager for the Geriatric Health Care for Homeless Veterans (Geriatric HCHV) program. This is a non-supervisory role responsible for providing specialized care and support to elderly individuals participating in the Geriatric HCHV program. This includes but is not limited to coordination of services, assisting with Activities of Daily Living (ADL’s), and ensuring that residents receive the medical, social, and other supports/resources they need to live independently. This position involves working closely with the LVN (Licensed Vocational Nurse) Medical Case Manager, other staff, residents, VA staff, other healthcare providers and community providers.
Job Duties:
- Maintain a calm, friendly and cooperative atmosphere within the program and with Veteran participants, and promote a clean, orderly and healthy environment.
- Assist Veterans with unit cleanliness and laundry to maintain a safe and comfortable home environment.
- Provide transportation and/or transportation scheduling in collaboration with the assigned Case Manager or Program Manager. May transport clients to Veterans Affairs, medical appointments, grocery store, or other places as required.
- Conduct appointment and medication reminders. Monitor resident medication.
- Connect residents to VA and community resources as needed to ensure residents’ ability to remain independently housed.
- Provide life skills education and assistance as needed.
- Encourage social interaction and participation in community activities.
- Collaborate with on-site case management staff to ensure coordination-of-care.
- Establish strong working relationships with partner agencies to ensure connections are made with residents as needed.
- Ensure accurate and timely client file documentation entry into agency Electronic Health Record (EHR)
- Perform other duties as assigned.
Educational Requirements
Associate of Science (preferred)
Bachelor’s degree in related field (preferred)
Qualifications:
Certified Nursing Assistant License in California
Basic Life Support Certification
Minimum five years' experience as a caregiver
Minimum of one year experience in healthcare
Minimum of two years’ experience working with residential milieus, senior and/or adults with disabilities.
Experience working with participants with mental health or substance use considerations preferred.
Proficient in MS Office applications including Outlook, Excel, SharePoint, and Teams.
Proven ability to work independently while exercising consistent, high-quality judgement
DMV record in good standing
Physical Requirements:
Requires the ability to occasionally bend, stoop, reach, lift, and move or carry office supplies and materials weighing 20 pounds or less. Finger dexterity required for computer keyboard operation. Ability to safely assist and transfer participants with limited mobility.
Work Environment:
This position reports to our location in Menlo Park, CA. Team-oriented office and participant residential environment. This is an in-person position and is not remote eligible.
Offering:
$30 $34 per hour, depending on qualifications and geographical location.
