Mosaic Pharmacy is a rapidly growing, closed-door pharmacy that provides multi-dose robotic packaging for over 9000 patients across the United States.


Mosaic Pharmacy Service provides comprehensive pharmacy care to medically complex and vulnerable seniors. We provide a patient-focused, pharmacist-driven care model in collaboration with health systems, assisted living facilities and health plans. Our goal is to help our patients feel better about the medications they take every day by simplifying medication regimens, improving adherence, and driving positive outcomes. Mosaic Pharmacy Service is seeking a highly competent, customer focused Staff Pharmacist for a new and exciting opportunity. The Staff Pharmacist will be working closely with the Pharmacist in Charge (PIC) to ensure safety, accuracy and a great customer experience.


Key Responsibilities:

  • Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities and assessing allergies
  • Dispenses medications by packaging, and labeling pharmaceuticals to meet all state and federal legal and regulatory requirements.
  • Evaluates and monitors drug therapy regimens for adverse effects, drug interactions and allergies
  • Counsels patients on their medications and offers expertise in the safe use of prescriptions
  • Provides direct supervision for Pharmacy Technicians and support personnel
  • Collaborates with clinical staff, providers and other health care professionals regarding any medication related issues
  • Complies with all applicable legal rules, regulations and procedures
  • Assist PIC with managing day to day pharmacy operations and workflows
  • Assist in motivating, developing and inspiring the pharmacy team to work together towards achieving Mosaic Pharmacy Service goals while ensuring quality, patient safety and satisfaction.

Requirements
  • Graduate of an accredited College of Pharmacy (PharmD is preferred)
  • Minimum of 5 years of Pharmacy experience. Leadership or Management experience preferred.
  • Licensed to practice in Virginia.
  • Working knowledge of Framework LTC and Parata (is preferred)
