Navigator Team Coordinator

Job Summary: 

Maple City Health Care Center (MCHCC) is a Federally Qualified Health Center dedicated to improving our community’s health by making quality comprehensive healthcare accessible to all. We strive for a healthy community where everyone is cared for.    

The Navigator Team Coordinator facilitates the work of the navigator staff. They support the mission and values of Maple City Health Care Center, Inc. by supporting patients and non-patients in accessing affordable health care and social services. We are looking for a talented and motivated individual who is passionate about health care and is eager to contribute to a collaborative team.   


Duties and Responsibilities:

  • Coordinates the work of the navigators 
  • Supervises members of the Navigator team 
  • Facilitates Navigator training 
  • Collaborates to create agenda and host team meetings 
  • Coordinates with Human Resources for new staff and exiting staff 
  • Maintains documentation of navigator workflows 
  • Represents Maple City Health Care Center as the team coordinator with community partners, including IPHCA and Goshen Health 
  • Participates in outreach activities to raise awareness about health insurance options and navigator services 
  • Assists patients and non-patients in accessing insurance information and coverage through Medicaid and the Marketplace 
  • Maintains certification as an Indiana Navigator and as a Certified Application Counselor 
  • Helps patients access in-house affordability resources 
  • Helps patients access Goshen Hospital Financial Assistance Program 
  • Other duties that support our shared work, as assigned 
  • Demonstrates effective interpersonal skills 
Requirements

Required Skills and Abilities:

  • Certification as an Indiana Navigator required  
  • Certified Application Counselor preferred; training provided  
  • Experience in cross-cultural settings  
  • Effective interpersonal skills 
  • Bilingual in English/Spanish required

Education and Experience:

  • Bachelor’s degree preferred, especially in social work or related field 
  • At least one year’s experience providing navigator and/or social services 
  • Leadership or managerial experience strongly preferred  


Requirements:

  • Ability to perform considerable time sitting and reaching.  
  • Transportation to travel between health center sites   
  • Ability to perform prolonged periods of time in front of a computer.

Salary Description
$22.36-$26.72 per hour
