Medical Data & Statistical Specialist
Venesco is seeking a Medical Data & Statistical Specialist to support the Naval Health Clinic at Oak Harbor, Washington.
This position will require you to be onsite at the Naval Clinic at Oak Harbor, WA.
The role shall provide analytical, administrative, professional, and technical support to include, but not limited to, the collection, analysis, and presentation of data and business practices to measure, monitor, and improve performance of the Medical Home Port program to include access, cost, enrollment, staffing, population, and staffing. This individual shall provide support to facilitate the overall objectives of the Naval Health Clinic Oak Harbor (hereafter referred to as "the MTF").
The Medical Home Port (MHP) program seeks to implement a new model of patient and family centered health care delivery for primary care that is team-based, comprehensive, and designed to fully meet the complete primary care health and wellness needs of patients. The contractor shall provide Health Systems Specialist (Data Analyst) services to meet workload requirements of all Medical Home Port teams at, belonging to, and associated with the MTF.
This role shall provide Health Systems Specialist (Data Analyst) support to include the collection, analysis and reporting of data from internal and external data systems, including designing reports and benchmarking results in a clear and concise manner, as well as aggregating, processing and interpreting report data for decision making in an effective and efficient manner and other electronic repository sources and analyze, present to department, directorate, and command.
Responsibilities:
- Track assigned tasks by priority and status as provided by the Director Health Services (DHS). Establish reporting mechanisms and perform analysis of activities and operations to determine progress towards stated goals and objectives.
- Ensure timely (daily, weekly or monthly depending on reporting system periodicity), readily accessible and clear patient centered data through comprehensive data evaluation, presentation, and database management. As new or revised guidance on primary care is released, contractor will assist in realigning the MHP and DHS practices as necessary.
- Participate in developing and implementing short and long-range plans with specific measurable standards, goals, and objectives for each clinic to ensure and enhance quality patient care and fiscally responsible business growth.
- Identify problems and independently utilize problem solving techniques to ensure the delivery of quality patient care.
- Submit recommendations for action and resolution to DHS leadership and respective Department Heads on identified problems.
- Participate in appropriate communications to patients using secure messaging system. Examples of such communication would include blast messaging about events such as school physical exams, breast cancer awareness month, and tobacco or targeted messaging to patients who are overdue for routine preventive services or disease management services.
- Maintain knowledge of the beneficiary population, optimization plan and resource consumption, managed care, population health, and empanelment practices.
- Identify new clinical and management requirements for the clinics; submit recommendations to MHP leadership to ensure necessary changes and adaptations are implemented.
- Maintain appropriate files as required by DHA, BUMED, the MTF, TRICARE Region, or The Joint Commission.
- Demonstrate effective collaboration, organization, and coordination of primary care operations.
- Promote staff development utilizing available resources. Assess plans, coordinate, and participate in scheduling of training and continuing education opportunities for clinic personnel.
- Provide reports to MHP teams and providers regarding medical record completion, coding completion and accuracy, timecard completion and accuracy, and other metrics of business related measures in an effort to improve performance.
- Check for certain open appointment types in Family Practice and Pediatrics Clinics (indicates cancelled appointment) at the beginning of the work day and each hour, or as frequently as possible all day. The contractor shall justify discovered cancelled appointment within the Electronic Health Record (EHR) and "patient cancel" as appropriate.
- On a daily basis prior to 0745, the contractor shall pull data from the EHR and compile the "DHS Report," which is disseminated to all Directorate of Health Services (DHS) Department Heads and Clinic Managers.
- While compiling the daily DHS Report, the contractor shall update the "Provider Kept Appointment Tracker" with how many appointment each provide actually kept.
- Prepare the TRICARE Operations Center (TOC) Daily Access Report. The contractor shall check the daily Acute Access data source to look for anomalies (more than one day). The contractor shall provide the report to the DMS Clinic Manager as requested.
- On the first workday of the week, the contractor shall prepare the Provider 3rd Next Available Report. This report is pulled from the EHR and counts each Providers' third next available routine appointment. The contractor shall pull the data from the EHR prior to 1200 to ensure the data is accurate.
- Check the weekly 24HR and FTR Access data source on the TOC to look for any anomalies in the Access data, more than 1 day for 24HR and 7 days for FTR. The contractor shall report the data to the DMS Clinic Manager as requested.
- Upon request, shall provide a report to 4th level Medical Expense and Performance Reporting System (MEPRS clinic) or other primary care MHP team level (rolling 30 day performance) regarding, but not limited to, the following metrics:
- *PCM (Primary Care Manger) continuity by name
- *PCM continuity by team
- *3rd next acute % met
- *3rd next acute average time to
- *3rd next routine % met
- *3rd next routine average time to
- *Emergency Room (ER) utilization/100 enrollees direct care
- *ER utilization/100 enrollees purchased care
- *ER utilization for AD Operational Forces cared for in appropriate 4th level teams
- *Indeterminate readiness (BSO-18 and shore based tenant command patients as appropriate)
- *Patient satisfaction with overall care (from The Monitor)
- *HEDIS quality metric performance
- *Enrollment per assigned FTE
- *Leakage of primary care workload to other teams, ER, purchased or direct care that could/should be performed by 4th level teams
- *Verification of schedules opening the EHR at least 28 days in advance
- Monitor template mix including types of appointments and % of each
- No show rates and appointment cancellations
- Incomplete encounters and un-coded encounters
- Compliance with data quality metrics
- Other metrics as developed locally by the clinical teams to help improve performance on Navy Medicine's strategic priorities 2.20. The contractor shall update monthly dashboard report for teams to the provider level (provider report cards).
- Analyze above metrics and consider suggestions for improvement in performance with leadership. The contractor shall provide support to command Dashboards (including the collection, analysis and reporting of data from internal and external data systems, including designing reports and benchmarking results in a clear and concise manner, as well as aggregating, processing, and interpreting report data for decision making in an effective and efficient manner. Systems include the EHR, Defense Medical Information System (DMHRSi), Command Management System (CMS), Tricare Operations Center (TOC), Template Analysis Tool (TAT), M2, the JOES, and disease management databases/tools such as ORYX (The Joint Commission's initiative to integrate performance measures into the accreditation process) , HEDIS (Health Plan Employer Data and Information Set), and any other databases implemented in the future containing information useful in managing healthcare team performance.
- Identify, extract, and analyze health care data to determine patterns in clinical care delivery at the local MTF level with capability to drill down to the 4th level MEPRS or practice level.
- When released by the developer, the contractor shall transpose the Medical Officer of the Day (MOOD) Watch Bill to the Schedule and Leave Tracker. The contractor shall transpose MOOD Watch Bill to 24/7 Comp Time Tracker.
- Pull data and compile the Access to Care (ATC) report for the ATC Meeting as required. The contractor shall submit the ATC Report to the DMS Clinic Manager.
- Communicate and coordinate on an as needed basis, but no less than monthly, with members of various departments throughout the MTF to facilitate dissemination of pertinent information to improve staff education, workflow and patient centered care as measured by specific performance metrics.
- Work to establish timelines with the clinics and assist with meeting goals and monitoring compliance TRICARE Access to Care standards.
- Utilize the command standard database and set of tools to obtain the data for reports to committees, departments, or command staff (which may at times include any Military Health System personnel).
- Assist the Director, Clinic Manager, and DHS Department Heads in preparing for professional presentations.
- Produce summary briefs for presentation to the Executive Steering Council (ESC), Region or other presentation venues as requested by the Command, Directors, DHS Clinic Manager, or MHP Department Head/Clinic Manager 2.30. The contractor shall help input and manage clinic schedules in the EHR as required/requested by local chain of command.
- Actively and frequently participate in and provide leadership for LEAN Six Sigma/Quadruple Aim Performance Plan (QPP) process improvement initiatives including facilitating discussions, recommending courses of action and supporting implementation of strategic goals, and business plans and practices in direct support of the MHP program. Such activities include, educating patients and staff on MHP tenants, workflow analysis, performance improvement, marketing, and special projects. The contractor shall plan and implement clinic based Performance Improvement I LEAN Six Sigma activities and share results for integration at all levels of the MTF.
- Provide support to Officers in Charge (OIC), Senior Medical Officers (SMO), Team Leaders, MHP Champions, Division Officers, Department Heads, Residency Directors, and Directors to assist in achieving their MHP mission as MHP practices mature or are created.
- Provide technical assistance, consultation, and training (primarily one on one or clinic/practice level, but also semi-annual command wide level) to staff members in the areas of data collection and analysis including statistical methods, analytical procedures, and statistical software programs. The contractor shall develop, analyze, and enhance Navy Medicine and DHA performance goals for individual providers and clinical and non-clinical staff.
- Assist the Database Administrator with the creation of SQL Server Reporting Services (SSRS) data models by identifying and locating data necessary for a given audience of customers consistent with the scope of this contract's provision of services.
- Develop lists and document libraries from the EHR and make that data accessible to staff members for demand and population health management.
- Create clear and comprehensible aggregate data presentations from data available.
- Establish regular meeting schedules for all levels of staff, including meeting format (including minutes) and report requirements.
- Provide a monthly report regarding status of projects to COR with a copy to leadership the last Friday of each month.
Qualifications:
- An associate’s degree or greater in a healthcare, business, or information management major.
- A minimum of 3 years of experience analyzing and presenting data.
- Experience using LEAN Six Sigma process improvement methodologies.
- Experience using Microsoft Office, particularly higher-level Microsoft Excel functions (pivot tables, pivot charts, etc.).
- Experience developing and training colleagues in individual and group settings.
- Skill in written and oral communication to prepare and present findings.
- U.S. citizenship is a basic condition for assignment. U.S. citizens who are also dual citizens are not specifically excluded from occupying either sensitive or designated IT positions, however, a dual citizenship status raises foreign influence and foreign preference concerns that will most likely prohibit interim assignment pending favorable investigation and adjudication of these issues. Eligibility will not be established for persons who hold a foreign passport.
- Current certification in American Heart Association Basic Life Support (BLS); American Heart Association Healthcare Provider Course; American Red Cross CPR (Cardio Pulmonary Resuscitation); or equivalent. Training shall be the responsibility of the contractor and is at no cost to the Government. Contract employees, not in possession of current certification, must acquire certification prior to initiating contract performance. Web based classes do not meet these standards.
HOURS OF OPERATION
Ø The Naval Health Clinic Oak Harbor Medical Services Directorate operates from 0700 to 1600 hours Monday through Friday (excluding Federal Holidays). Performance is not expected to exceed 40 hours per week.
Ø All closings for severe weather or other emergency will be at the sole discretion of the government. The contractor shall maintain the required service schedule unless the COR notifies the contractor that weather conditions or an emergency authorizes a closing. The contractor will receive guidance from the COR of the MTF protocol for closings.
Ø Telework: Teleworking on the part of the company is voluntary. Teleworking shall not result in an increase in contract price. The Contractor is responsible for continuity of performance in accordance with the terms of the contract. Telework shall be coordinated between the Contractor's Program Manager and the Government and subject to approval by the Contracting Officer and/or the Contracting Officer Representative. Any equipment provided by the Government for telework purposes will be treated as Government Furnished Equipment. The contractor shall monitor worker access, performance, and duty hours. The Contractor shall have written Policy Guidance established and provide adequate oversight of work products to ensure contract compliance while teleworking which will be provided to the Government.
