Ascend Medical is seeking a dedicated Nurse Practitioner or Physician Assistant who is passionate about building meaningful relationships and providing compassionate care to assisted living residents. Key Responsibilities:
· Comprehensive Assessments: Conduct initial and ongoing evaluations for new and existing residents in collaboration with the assisted living facility’s healthcare team.
· Care Planning: Develop individualized plans that address each resident’s physical, emotional, and social needs, partnering with families and other clinicians.
· Medication Management: Oversee prescriptions and adjust treatment regimens to ensure efficacy, safety, and timely administration.
· Transitions of Care: Coordinate hospital admissions, discharges, and referrals to specialists or higher levels of care, ensuring seamless support for residents.
· Advanced Care Planning: Guide discussions around advanced directives and long-term care strategies to promote resident comfort and autonomy.
· Patient & Family Education: Communicate effectively and empathetically with residents and their families, ensuring they understand treatment plans and care goals.
· Collaboration: Work closely with assisted living staff and community resources to maintain a high standard of care, resolve issues promptly, and foster a supportive environment.

Qualifications:
- Licensed Nurse Practitioner or Physician Assistant with 3+ years’ clinical experience, preferably in assisted living or geriatric care.
- Master’s degree from an AMA accredited school.
- Active, non-probationary, unrestricted Georgia State PA or NP License
- Active unencumbered DEA License for Georgia OR eligibility to obtain
- Ability to work in North Metro Atlanta area- Woodstock/Canton area.
