Job Summary:

To provide overall supervision and supportive services to residents in the New Opportunity Home program.

   

Work Schedule Demands:

  • Full-time, part-time and relief positions with rotating shifts to include evenings, nights, weekends and holidays. 
  • Due to the nature, responsibility and demands of this job, the employee must be flexible with their schedule and will be expected to cover shifts in the event the next shift staff are unable to relieve. 
  • May be required to work at any CCAP location. 


  

KEY RESPONSIBILITIES:

  • Responsible for the overall care of the residents (safety, health, hygiene, education, social and emotional wellbeing). 
  • Participate in providing support for each resident in compliance with their RI Works activity and adhering to the guidelines of the contract.
  • Provide supportive services and advocacy in order to promote positive outcomes.
  • Assist in obtaining relevant assessment data (school records, assessment summaries and other documents).
  • Supervise the behavior and activities of residents and to participate in identifying personal and adjustment issues of residents. 
  • Communicate with Program Coordinator and RI Works Case managers on a weekly basis regarding their activity. ( RI Works components such as supportive services, Teen family development, Vocational training and job readiness)
  • Implement and supervise the Adult Daily Living program activities.
  • Monitor self-medication administration when needed.
  • Provide individual and/or group crisis intervention and initiate appropriate behavioral management techniques.
  • Participate as a team member in the implementation, monitoring and evaluating of program planning of the residents.
  • Assist in the transportation of residents to appointments as needed. 
  • Organize and maintain appropriate weekly progress notes, logs and/or other data
  • Collaborate as a member of the staff team in enforcing all house regulation and procedures within the guidelines of the overall program philosophy 
  • Participate in all case conferences when needed.
  • Attend trainings for mandatory CPR and other trainings scheduled.
  • Attend any relevant program meetings.
  • Attend staff training, schedule and attend supervisory meetings with Program Manager
  • Assist in educating the community about programs within CCAP
  • Participates in agency team meetings
  • Other duties as assigned


ADDITIONAL SKILLS:

  • Knowledgeable of family systems, short- and long-term treatment
  • Organizational skills
  • Competent writing skills, daily logs and reports as needed.
  • Competent to implement case plan within time limits 
  • Necessary verbal skills; case presentations, etc.
  • Knowledge of child development
  • Works well independently and as a team member
  • Manages conflict and performs well in crisis situations 
  • Willingness to participate in training and continuing education 
  • Willingness to engage in self-evaluation and to establish goals for professional development
Requirements

REQUIRED QUALIFICATIONS:

  • High school diploma, GED and/ or Associate’s degree; preferred
  • Transitional and/or supportive housing work experience with infants, children and families; preferred
  • Familiarity with DHS and RI Works program; preferred
  • Documented experience working with children and their families
  • Complete all required background checks
  • Valid Driver’s License and proof of Liability insurance
  • CPR/First Aid Certificate upon hire or within 90 days
  • Restraint training or willingness to obtain
  • Provide/maintain all required immunizations and/or vaccinations

Has access to program case management records.  Maintains the confidentiality of all materials in accordance with State and Federal laws and CCAP policies and procedures.

While performing the duties of this job, the employee is regularly required to sit and to talk to hear.  The employee frequently is required to use hands, fingers, handle or feel objects, tools or controls and reach with hands and arms.  The employee is occasionally required to stand and walk.  The employee must occasionally lift and / or move up to 20 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.  The environment is a shared living facility and has a separate office and space for staff.   Potential exposure to blood borne pathogens, respiratory diseases and other infectious diseases, and other biohazardous material requiring universal standard precautions.  This position requires the individual to have their own transportation, a current unrestricted license to drive, documented auto insurance and the individual must be willing to work in schools and the community.
