Community Health Representative
Description

Position Purpose:

The Community Health Representative (CHR) I – Transporter will provide rides to medical appointments for those clients who cannot perform that for themselves for whatever reason. CHR I may also pick up and deliver medications, with the exclusion of controlled substances, to clients as necessary. The CHR I will communicate with the Manager any other needs the client may express or the CHR I notices. CHR I will also assist CHR II & III with community outreach such as health fairs, health screenings, shot clinics, flyer distribution, or other outreach events. CHR I will be responsible for assisting any other HHS employees with clients, outreach, or events within the scope of grant, as well.

Essential Duties and Responsibilities:

  • Transport clients to medical appointments locally and within a 90-mile radius
  • Document client interactions and vehicle information 
  • Pick-up and deliver medication orders, as necessary, excluding controlled substances
  • Assist in community development through outreach, education, health-related events
  • Other duties as assigned

Education and Experience:

  • Possess a high school diploma or equivalent
  • Completion of health-related training preferred

Knowledge, Skills, and Abilities:

  • Must have excellent people skills
  • Possess a valid driver’s license with an excellent driving record
  • Offer some flexibility
  • Must be self-motivated and able to identify needs as they arise.
  • Ability to communicate effectively with diverse populations, both oral and written
  • Ability to maintain confidentiality and adhere to ethical standards

Certifications:

  • Valid Driver’s License and Insurance
  • Completion of health-related training preferred

Requirements

Work Requirements:

Position requires driving and transporting clients to appointments sometimes out of town. Position involves some physical demands such as walking and standing, pushing and pulling, bending and lifting up to 50 lbs. May have to offer mobility assistance to some clients. Position also involves occasional need to work a flex schedule that may include coming in 1 ½ hours before 8 am and/or working 1 ½ hours past 4:30 pm. Position requires proper maintenance and cleanliness of vehicle. Work is generally performed in various environments that may offer varying levels of lighting, air quality, noise, and / or temperature.
