As a human service agency and Supported Living Program, Community Integrated Services (CIS) is committed to serving adults with developmental disabilities and behavioral issues—and helping them thrive. Many of the individuals CIS serves are challenged by severe behavioral and mental health issues.
Enhance the coordination of supports available to clients, including health and safety;
· Increase the provision of support to all houses as directed
· Be responsible for taking action when the on-call or program coordinator contacts you during your scheduled shift to give directives as to what is needed and where to go
· Communicate regularly with Program Managers for the houses you support regarding client issues and DSP interactions
· Communicate regularly with Managers regarding health and safety issues for houses as assigned
· Attend monthly house team meetings where recently supported and attend weekly meetings with program managers to discuss issues
· Follow the developed Individual Instruction and Support Plan (IISP), Positive Behavior Support Plan (PBSP), Exceptions To Policy (ETP), and any restrictive or Crisis Intervention Response Procedures (crisis plans)
· Assignments involve work of a confidential or complex nature, necessitating exposure to sensitive information and contacts requiring considerable discretion, judgment, tact, and diplomacy
· Shift will require at least one weekend day per week
· Fill in shifts as needed to cover houses (e.g.; while another staff takes a client to a medical appointment and/or therapy, call-outs, break coverage, etc)
· Review client documentation for completion and accuracy
· Review client meds for expiration dates, need for ordering refills, etc.
· Schedule fire drills for houses and observe client/employee responses
· Other duties as assigned
