Community Medical Services (CMS) is hiring a Correctional Health Liaison responsible for seeing referred and assigned clients within Correctional Health Systems and maintaining caseload in compliance with the Substance Use Treatment Program. The Correctional Health Liaison reports directly to the Correctional Health Supervisor. This position works closely with assigned Clinic Managers, Operations Manager, and Director of Community Programs, Regional Director, and Clinic Managers to assist with appropriate coordination of Correctional Health and/or DCS relations. Maintains accurate documentation for assigned clients and provides substance abuse rehabilitation, support, and education services to assigned patients in compliance with the Substance Use Treatment Program and philosophy established by the program committee.

Responsibilities:
- Works in conjunction with System Partner Programs Manager and Clinic Manager/s to maintain an average high needs case load of 40 clients and to maintain a 45% productivity ratio.
- Correctional Health Liaison works with clients receiving services from Correctional Health, Department of Child Safety, and Criminal Justice Systems.
- Provides documentation of client encounters.
- Maintains appropriate coordination of care with all systems and provides documentation as needed/ requested from multiple system partners.
- Attends required meetings: Clinical staff meetings, clinical supervision, workshops, seminars and training, etc. as assigned by the System Partner Programs Manager, Director of Community Programs, Regional Director, Clinic Managers, Clinical Compliance Coordinator and/or the Chief Medical Officer. May provide in-service trainings to staff.
- Community Based Services of travel time, waiting for system partners, and non-billable services during community-based practices. Providing program information and assessment information to non-clients.
- High needs demographic with above average level of coordination of care. Coordination with PCP, OBGYN, Correctional Health Services, Drug Court clients, family court, Department of Child Safety, and other community partners. Acts as a supporting contact of the System Partner Programs Manager.
- Substance abuse rehabilitation services and education of assigned caseload in compliance with the Substance Abuse Treatment Program and Philosophy as established by the program committee. Provides substance abuse counseling and education services. Completes intake and orients client to treatment program. Develops and implements behavioral health service plans and reviews. Documents client progress in clinical records. Provides coordination of care between mutual providers and act as a liaison between clients and outside agencies and programs. Provides appropriate treatment and service referrals as needed.
Qualifications:
- Required to pass government background check and meet qualification to receive badge needed to enter jails/DOC
- Behavioral Health Technician or Paraprofessional as defined by ADHS
- Experience in substance use rehabilitation and Medication Assisted Treatment is desirable
- Basic computer knowledge is necessary
- Bilingual a plus
- (2) year(s) of experience in opioid treatment program preferred
- (1) year experience in corrections or criminal justice related position
- Required sensitivity to the needs of those who are chemically dependent
- Culturally competent and sensitive to client individualized treatment needs
- Ability to participate as an effective and efficient team member
- Ability to communicate clearly and effectively
- Ability to interact with clients in a friendly, helpful, and courteous manner
- -Ability to resolve client altercations and manage crisis episodes
- Ability to identify and access community resources
Tools and Equipment Requirements
- The ability to use a phone, computer, printer, and copier is required.
- Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint.
- The ability to use the internet and various web browser software is required.
Physical Working Conditions and Office Setting Description
- Requires sitting and standing associated with a normal office environment.
- Manual dexterity using a calculator and computer keyboard.
- Requires prolonged sitting, standing, frequent bending, stooping, or stretching.
- Some lifting may be required.
- Frequent and prolonged typing and frequent and prolonged operation of computer, keyboard, and telephones required.
- Requires occasional use of fax machines, telephones, copiers, and other office equipment.
- Employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and or move up to 50 pounds.
