Position Summary:

We are seeking a compassionate, tech-savvy, and highly motivated Nurse Practitioner (NP) to join our growing healthcare team. This hybrid position allows you to deliver high-quality, patient-centered care primarily through telehealth (90%), with minimal in-person visits (10%) for essential physical exams, procedures, or follow-ups.

The ideal candidate is comfortable using telehealth platforms, confident working autonomously, and dedicated to improving access to care for patients across a wide geographical area.


Key Responsibilities:

  • Conduct comprehensive assessments, evaluations, and follow-ups via secure telehealth platforms.
  • Diagnose and treat acute, chronic, and preventive conditions within scope of practice. 
  • Develop and manage individualized treatment plans and prescribe medications when appropriate. 
  • Coordinate in-person care as needed for procedures, exams, or urgent follow-up. 
  • Educate patients on treatment plans, medications, lifestyle, and wellness strategies. 
  • Collaborate with multidisciplinary care teams for continuity of care. 
  • Maintain accurate and timely documentation in the EMR system. 
  • Provide a seamless patient experience, focusing on accessibility, empathy, and clinical excellence.
     
Requirements

Qualifications:

  • Current and valid Nurse Practitioner license in [State] (additional licenses a plus).
  • Board certified as FNP, AGNP, PMHNP, or equivalent (based on specialty needs).
  • Minimum 1-2 years of NP experience preferred; telehealth experience highly desirable.
  • Proficient in using EMRs and telehealth technologies.
  • Strong communication, clinical decision-making, and patient education skills.
  • Must have a reliable internet connection and private work-from-home setup for telehealth visits.
  • CPR/BLS certification (as applicable).
