Position Summary:
We are seeking a compassionate, tech-savvy, and highly motivated Nurse Practitioner (NP) to join our growing healthcare team. This hybrid position allows you to deliver high-quality, patient-centered care primarily through telehealth (90%), with minimal in-person visits (10%) for essential physical exams, procedures, or follow-ups.
The ideal candidate is comfortable using telehealth platforms, confident working autonomously, and dedicated to improving access to care for patients across a wide geographical area.
Key Responsibilities:
- Conduct comprehensive assessments, evaluations, and follow-ups via secure telehealth platforms.
- Diagnose and treat acute, chronic, and preventive conditions within scope of practice.
- Develop and manage individualized treatment plans and prescribe medications when appropriate.
- Coordinate in-person care as needed for procedures, exams, or urgent follow-up.
- Educate patients on treatment plans, medications, lifestyle, and wellness strategies.
- Collaborate with multidisciplinary care teams for continuity of care.
- Maintain accurate and timely documentation in the EMR system.
- Provide a seamless patient experience, focusing on accessibility, empathy, and clinical excellence.
Qualifications:
- Current and valid Nurse Practitioner license in [State] (additional licenses a plus).
