InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor.
InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary:
InterCommunity HealthCare is currently seeking an Addiction Medicine Coordinator to join our team. The ideal candidate will be responsible for a combination of grant coordination, medical administrative tasks, and recovery support specialist duties. This role requires a proactive, detail-oriented individual with excellent organizational and communication skills. The Addiction Medicine Coordinator will ensure smooth operations and support healthcare professionals.
Patient Support:
- Assist patients in navigating their recovery process, providing guidance and resources as needed.
- Coordinate and schedule appointments with healthcare providers and support services.
- Act as a liaison between patients and the healthcare team to ensure continuity of care.
Grant Management:
- Maintain up-to-date documentation for the addiction medicine expansion grant, ensuring compliance
- with all requirements.
- Collaborate closely with the Project Director, Grants Manager, and Practice Manager to track grant
- progress and ensure a timely meeting of due dates.
- Collect and analyze data in accordance with the Government Performance and Results Act (GPRA) to set
- objectives, measure outcomes, and facilitate reporting.
Clinical Duties:
- Perform clinical responsibilities, including rooming patients, gathering medical histories, conducting
- vital sign assessments, and reconciling medications.
- Ensure accurate and timely documentation of patient interactions and clinical findings.
Data Collection and Reporting:
- Gather and compile data for performance measurement and goal setting.
- Prepare regular reports on grant outcomes and patient progress for internal review and submission to
- external stakeholders.
Team Collaboration:
- Collaborate with healthcare providers, therapists, and administrative staff to provide comprehensive
- patient care support.
- Participate in staff meetings and training sessions to enhance team performance and patient services.
- Gathering patient medical history and updating electronic health records
- Collaborating with healthcare professionals to ensure efficient patient care
- Maintaining cleanliness and organization in patient care areas
- Maintain accurate and up-to-date documentation of patient assessments, care plans, progress notes, and services rendered. · Ensure compliance with regulatory requirements and healthcare policies and procedures. · Adhere to all federal and state regulations and guidelines. · Always maintain patient confidentiality.
- Perform other duties as assigned.
Schedule:
Monday - Friday 7:30 AM to 4:00 PM

- Strong organizational and communication skills

- Knowledge of medical terminology, procedures, and medications
- Proficiency in using electronic medical records and other healthcare software.
Education &/Or Experience:
- Associate’s degree, medical assisting, or equivalent experience.
- Holds a valid registered medical assisting accreditation preferred.
- At least 2-3 years of experience working in a medical setting.
- Bilingual English to Spanish preferred
