Job Summary & Deliverables
The Social Service Worker provides therapeutic behavior management, case management, and other services as defined by licensure to ensure excellence in client care, fiscal management, and regulatory compliance.
- Provides therapeutic behavior management as prescribed by care plan
- Assists therapist as needed to develop and review safety/crisis plans and care plans
- Coordinates services among internal/external agencies and internal/external providers
- Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
- Ensures continuity of care
- Meets required productivity standards and other organizational expectations
Education
- Bachelor’s degree in Social Work or related field
Experience
- Internship experience as required for SSW license
Licenses/Certifications
- SSW licensure in state of practice
- CPR certification
- Valley de-escalation certification
