Position Summary: The Ramsey County Children’s Mental Health Case Manager will focus their services on families with children who have serious mental health needs navigate the mental health system. They will help children and their families organize therapeutic and other culturally sensitive supportive services that address the child’s emotional and behavioral issues and related social, recreational, health, educational and vocational needs this includes developing a treatment and crisis plan. The case manager (Rule 79 Case Management) will work in collaboration with the families to develop a supportive team made up of family, professionals, and community members that will assist parents in meeting the needs of the child. Case managers will provide referrals to community resources and assist parents in advocating on behalf of their child.

- Responsible for long-term case management of children & young adults ages 5-21 who have significant mental health diagnoses.
- Collaborate with families to develop functional assessments, treatment and crisis plans with other services providers and the child’s family to best address the child’s needs.
- Assesses a child’s changing needs, monitor the child's progress toward achieving the outcomes, report progress and revise as appropriate based on the child's progress.
- Create a supportive team of family, professionals, and community members that will be involved in the treatment of children and coordinate services.
- Links the child & family to appropriate community resources, monitors the effectiveness of planned services and assists parents in advocating for their child’s mental health needs.
- Complete monthly face-to-face visits with children and/or families and provide informal counseling, crisis management and parenting skills education as needed.
- Actively participate in discharge planning for the child and, to the extent possible, coordinate the services necessary to assure a smooth transition.
- Follow Ramsey County’s policies & procedures.
- Responsible use of the county database (SSIS), attending collaborative unit meetings, placement meetings, monthly supervisor meetings, and county all staff training.
- Responsible for confidentiality of client records including storage and transportation.
- Work in the consideration of the Indian Child Welfare Act, & Minnesota Indian Family Preservation and Minority Heritage Act.
Qualifications: The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Bachelor’s degree in a behavioral science/related field or minimum of 3 years’ experience. Master’s degree and/or licensed mental health professional (LMFT/LPC/LICSW/LP) or working towards licensure preferred.
- At least 2 years of case management or direct work with high-risk populations and special needs children preferred.
- Must have a understanding of children’s mental health case management, processes, diagnoses, symptoms, and methods of treatment
- Skilled in the process of identifying and assessing a wide range of client’s needs.
- Must be knowledgeable about community resources and how to use the resources for the benefit of the child and the child’s family.
- Ability to work with youth and families of various economic or social backgrounds, and with various medical and mental health conditions.
- Must demonstrate and promote a chemically healthy lifestyle.
- Must be mindful and sensitive to the special needs/issues of American Indian youth and their families.
