Job purpose

As a QMAP, your primary responsibility is to provide personal care and to administer medications, as prescribed and within the scope outlined in the relevant regulations, to the residents in our community. If you have not received your QMAP, please only perform caregiving duties. Once you have received your QMAP and have gone through QMAP training and had a satisfactory shadowing review by a QMAP Trainer/Coordinator, you will perform the QMAP duties as outlined in this job description. Delegated responsibility to perform duties as stated in The Community Policies and Procedures and in compliance with all relevant rules and regulations.

Schedule

Must be able to work 6a-2p and/or 2p-10p on Saturday and Sunday. Additional days and hours available.


QMAP duties:

  • Administers medications to residents based on the seven Rights; Person, Right Drug, Right Dose, Right Route, Right Time, Right Documentation, Right of the Client to Refuse
  • Reorders medication in a timely manner
  • Follows Community Policies and Procedures on medication administration 
  • Keeps medication cart clean and well stocked, reporting any out-of-stock medicine and refused medicine to the appropriate Community resource
  • Charts all medication administrations in a timely and accurate manner
  • Adheres to the QMAP Scope and Standards for practice in Colorado Qualifications and Experience
  • Minimum High School Diploma or GED preferred
  • Eager to Learn and to be a Part of a Strong Team
  • Pass Background Check and Drug Tests
  • Pass Routine TB Test
  • Able to use general office equipment and computer systems required
  • Must demonstrate dependable, regular attendance
  • Must be able to communicate and function productively on an interdisciplinary team
  • Has familiarity and willingness to work with elderly population
  • Receives and maintains CPR/First Aid certification
  • QMAP must complete Colorado QMAP class, pass written and practical test, and has never had a professional license or certificate revoked by a state or any other jurisdiction. Must have a record of satisfactory med pours

Schedule:

Must be able to work 2p-10p on Saturday and Sunday. Additional days and hours available.

Working conditions:

May be exposed to infectious waste, diseases, conditions, etc. Please wear Personal Protective Equipment (PPE) provided.

Physical Requirements:

Must be able to meet physical demands of lifting and moving over 50 pounds, sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours. Moving at a high pace for long periods of

Hours and Dress Code:

Non-exempt, hourly position. Must be available on weekends. Scrubs are provided & required.

Responsibilities:

  • Administering daily medications to residents according to our high standards for safety and effectiveness.
  • Observing and documenting residents' conditions and reactions to medications.
  • Cooperating with other healthcare providers to ensure the implementation of comprehensive, individualized care plans.
  • Participating in residents' wellness activities and contributing to their quality of life.
  • Complying with state laws and facility guidelines concerning medication administration.
  • Assist residents with ADL’s.

Qualifications:

  • A valid QMAP certification.
  • Experience in assisted living, long-term care, or memory care settings is highly desirable.
  • CPR
  • Empathy for and an understanding of the needs of seniors, and those living with dementia.
  • Strong communication and interpersonal skills.
  • Willingness to work flexibly in a dynamic, fast-paced environment.
  • Excellent observation, problem-solving, and decision-making skills

Requirements
