As the largest Federally Qualified Health Center (FQHC) in Denton County, we are currently in need of an Advanced Practice Provider (FNP or PA-C) who is seeking to provide exceptional patient care to a diverse population of patients and medical needs. As a community Health Center, we strive to “close the gap” by leveraging our clinic and community resources to provide high-quality, evidence-based care to those in the community who need us most. Focusing on continuity of care, we treat both acute and chronic conditions, including infectious diseases. We also provide excellent primary medical care for children and adults with a focus on establishing a medical home for our patient’s comprehensive healthcare needs.


Our healthcare providers operate at the very highest level of their expertise as they provide comprehensive, team-based care in a fast-paced environment within a non-profit, community healthcare setting. Our providers love HSNT because we treat patients from all walks of life--regardless of socio-economic status--for a wide range of acute and chronic physical, mental, and social health conditions and in collaboration with a team of medical professionals.


HSNT is seeking providers that are passionate about providing care, education, and resources in a community health setting to patients and families that have very limited options for their medical needs; and those providers that are ready to embrace the challenge of maximizing their already existing knowledge base and experience in chronic conditions.

Requirements

A day in the life of our Advanced Practice Provider may look like this:


  • Reviews initial intake, health history, and laboratory test results, performs physical examination, and designs treatment plans catered to client’s health status to provide appropriate interventions and routine health maintenance.
  • Provides additional assessments and evaluation of drug therapies and treatment regimen during routine follow-up clinic visits, ensuring clients continue to receive effective, efficient primary health care.
  • Maintains open consulting relationship with the physician ensure delivery of the most effective medical care.
  • Educates client on medication regimens and drug adherence.
  • Provides ongoing primary medical care to patients, including medical history, evaluation, appropriate treatment and give referrals to specialists for needs that exceed the scope of primary care.
  • Maintain client records in compliance with HIPAA and agency standards.
  • Provide ongoing guidance and supervision of medical care of clients to other clinical providers.
  • Adhere to all agency and clinic policies and procedures.
  • Participate in quality management studies.
  • May perform other duties as assigned by supervisor.


Education and Experience:


We ask our provider candidates to possess a minimum of:

  • Master’s level in related discipline.
  • Licensed Advanced Nurse Practitioner or Certified Physician Assistant in the State of Texas.
  • 3-years+ of post-graduate clinical experience in a Nurse Practitioner or Physician Assistant highly preferred.
  • Strong communication skills and professional, compassionate approach in practicing medicine.
  • Ability to relate to individuals from a variety of ethnic and cultural backgrounds, including various sexual orientations.
  • Bilingual (medical Spanish proficiency) highly recommended.
  • Basic EHR proficiency (we use eClinical Works).
