Description

The Clinical Outreach Coordinator is responsible for traveling to community events in Marion, Morgan and Monroe counties to perform health screenings (e.g., blood pressure, glucose, vision, BMI, etc.) and serve as a brand ambassador for HealthNet, its mission, and service offerings. Must be willing to drive the mobile unit if needed (training provided).


Patient/Participant Care

  • Serve as a brand ambassador for HealthNet, its mission, and service offerings.
  • Perform health screenings (e.g., blood pressure, glucose, vision, BMI, etc.) at community outreach events, with an emphasis on the mobile unit.
  • Educate participants on screening results and provide guidance on next steps.
  • Connect individuals to follow-up care, including referrals to healthcare providers and community resources.
  • Work with community organizations to schedule and promote mobile unit events.
  • Assist in planning and setting up outreach activities.
  • Build relationships with community partners to enhance healthcare access.
  • Ensure the mobile unit is fully stocked with necessary medical supplies, outreach materials and educational resources.
  • Maintain cleanliness and organization of the mobile unit.
  • Document patient interactions and screening results following compliance guidelines.
  • Track outreach metrics and prepare reports on program impact.
  • Apply a minimum of one health literacy method when communicating with patients/clients. 

Administrative 

  • Assist in planning and implementation of special projects and perform other activities as assigned.
  • Attend all required meetings, case conferences, and trainings as assigned by director.
  • Immediately report problems or unusual occurrences to immediate supervisor, or other HealthNet leadership as appropriate.

Requirements

Knowledge, Skills, Abilities 

  • Currently not sponsoring work visa. 
  • Requires completion of high school diploma or GED. 
  • Requires successful completion of an accredited educational program for medical assisting or experience working as a Community Health Worker.
  • Requires Basic Life Support certification through the AHA within one month of hire. 
  • Requires knowledge of various standard laboratory and medical procedures.
  • Requires knowledge of ICD-9 and CPT coding.
  • Bilingual skills are a plus.
  • Requires Valid Drivers License and reliable transportation.
  • Experience in community outreach, public health, or patient advocacy.
  • Understanding of data entry and basic math skills.
  • Competency with equipment and medical information pertaining to health screenings.
  • Requires computer experience utilizing Microsoft products. Additional experience with Electronic Medical Record systems preferred. 
  • Requires ability to accurately collect patient information and provide confidentiality.
  • Requires ability to meet standard if completing lab training or competencies. 
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively in diverse communities.
  • Present dedication and drive toward job.

Physical Requirement

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 20-30 pounds at times.
