The Bereavement coordinator is responsible for overall management of a Notre Dame At Home- Hospice, Palliative Care and Pedi Pals bereavement program. Responsibilities include the planning, implementation, maintenance and provision of services necessary to patients, identified caregivers, staff, and the community at large. Notre Dame At Home’s bereavement services extend for the time period of the patient’s admission to hospice services until at least thirteen months (24 months for pediatric deaths) after the death of the patient.

· Preferred degree in bereavement counseling, social work, or related field required.
· Training or experience in spiritual and psychosocial dynamics of the bereavement process, support group activities, and related program services.
· Ordained clergy, or spiritual education.
· Experience in program development, implementation and management.
· Experience in community education.
· Excellent organizational skills.
· Ability to work with members of the Interdisciplinary Team
· Strong background in mental health/counseling and experience in delivery of health services preferred.
· Ability to conduct assessment of patient/family bereavement risk.
· Ability to identify and prioritize problems with appropriate goal formulation and conduct required interventions.
· Demonstrated leadership capabilities with the ability to relate well to staff and other professionals.
· Valid state driver’s license and reliable automobile with proof of current automobile insurance
· Expected to participate in appropriate continuing education as required to maintain licensure.

Schedule:
· Typical workday hours are Monday through Friday from 8:30 AM to 5:00PM; this is subject to change based on patient/family needs.
